Coleman Adult Day Services is the alternative to home health care and nursing homes in Portage County. We assist family members and caregivers in keeping their loved ones, whether it be someone with a developmental disability, someone with Alzheimer's Disease, Dementia or another cognitive disorder, in their comfortable home environment for as long as possible. We do this by offering a progressive

medical model of health care at an affordable rate. Direct care is provided by our Caregivers with a 6:1 client to staff ratio. Our modern facility is centrally located in Portage County. We provide:- Mental and physical stimulation through structured activities planned by our Activity Director- Respite care for family members which decreases “Caregiver Burnout”- Nursing services with individualized care plans- Personal care, bathing and incontinence care- Daily breakfast, lunch and an afternoon snack (accommodations can be made for special diets)- Transportation to and from the facility can be arranged.
