ClearPath Healthcare

ClearPath Healthcare While our name has changed, our dedicated team, our commitment to 24/7 care, and our deep roots in Central Oregon remain the same.

Offering hospice care, transitions, veterans support, bereavement, and now in-home primary care throughout Central Oregon. Hospice care is designed to focus on quality of life for the terminally ill patient. Our goal is to enable patients to be as comfortable and as free of symptoms and pain as possible, so that they live each day as fully as possible. We believe in treating the patient holistical

ly, honoring their life and their wishes. Our philosophy is to provide support for the patient’s emotional, social, and spiritual needs as well as medical symptoms management as part of treating the whole person. We incorporate a small team approach in providing compassionate personalized care for each patient. Our primary focus is our patients and their families, who we serve with dignity and respect. We believe that our patients have a right to participate in all decisions about their care. We offer suggestions based on sound medical practices but it is our patient’s life and ultimately our patient’s choice as to what will help them on this very personal journey.

We want to sincerely thank the talented women of the High Desert Quilt Guild for donating their handmade lap quilts. Our Community Engagement Manager, Lauren, and Volunteer Coordinator, Jody, were grateful to meet these creative members and accept their wonderful gifts.

Each quilt is one-of-a-kind and brings comfort and warmth to our patients. At ClearPath Healthcare, these thoughtful gifts are an important part of the care we offer. We truly appreciate the time and effort the High Desert Quilt Guild puts into supporting our mission.

Our team works hard each and every day to deliver the highest level of care to our hospice patients. We are absolutely thrilled to be one of two hospice providers in Central Oregon with a 5-star rating on Medicare.gov.

This rating comes from family caregiver surveys in which patients' family and friends are asked whether they would recommend us to someone else.

100% said yes!

That is no easy feat, and we are truly grateful for the ability to care for so many wonderful humans!

Are you so busy caring for a loved one that you’ve missed your own check-ups? You aren’t alone.

Caregivers often go without necessary primary care because they can’t leave their loved one's side. At ClearPath Healthcare, we believe your health is just as vital.

Our In-Home Primary Care service is available for both the homebound patient and their caregiver. We bring board-certified expertise (led by Taylor Garr-Jones, FNP-C) to your living room, so you don't have to choose between your responsibilities and your well-being.

Your health matters, too. See how it works: https://f.mtr.cool/sdeatxkczc

Congratulations to our Giving and Outreach Specialist, Stephanie, on her recent induction into the Kiwanis Club of Sisters! We are proud to have her representing ClearPath Healthcare in such a dedicated service organization.

At ClearPath, we believe in being active participants in the neighborhoods we serve. Stephanie will be working alongside fellow Kiwanis members to support local youth and community programs throughout the area.

Did you know that Hospice of Redmond has been a steady presence for Sisters' families since 1979? As we continue our mission under the ClearPath Healthcare name, our commitment to this community remains as strong as it was over forty years ago. We look forward to many more years of service and partnership in Sisters.

Did you know that any veteran's name can be added to the Wall of Honor, the only one of its kind in Central Oregon?

This stunning monument honors any veteran with ties to our beautiful State of Oregon.

There is room for additional names on our Wall of Honor as each panel is installed independently once 61 names are collected.

Choosing hospice is a deeply personal decision, and Central Oregon is fortunate to have many dedicated providers. At ClearPath Healthcare, we are proud to serve as a community-based nonprofit. For us, this means every resource is reinvested right here at home, funding unique local programs like Camp Sunrise and grief support. It’s our way of ensuring that the heart of our mission remains rooted in the neighbors we serve.

We tried to narrow down our favorite moments from April (so far!) but there were just too many. The last few weeks have been a whirlwind of moments which remind us exactly why we do what we do.

We had our official ribbon cutting for our new name and In-Home Primary Care service with the Redmond Chamber of Commerce, as well as a community open house. It was a proud moment for our incredibly hardworking team: one which represents years of dreaming how we can better serve our community right where they are most comfortable- at home.

Shortly after, we had the humbling experience of hosting a "Welcome Home" Vietnam Veterans Day event at our local VFW. Getting to thank our local heroes for their service is incredibly important to us, and this event could not have happened without a lot of volunteers, donations and support from the community.

We wanted to share a look back at these two special days, but it is a lot of photos. Each one represents a face, a story and a connection that makes our work at ClearPath so vital.

Thank you for trusting us for being a part of your lives and welcoming us into your homes. We are so grateful to be a part of this community.

Meet Taylor Garr-Jones, FNP-C. As a Redmond local, Taylor knows that for many in our Central Oregon community, the "simple" act of getting to a doctor’s office is anything but simple.

In this video, Taylor shares why she’s passionate about bringing primary care directly to the doorsteps of our geriatric neighbors. By eliminating the stress of travel, we’re making high-quality medical care accessible from Southern Bend to Madras and everywhere in between.

"Our mission is to ensure that no one in our community is left behind just because they can't make it through a clinic door." — Taylor Garr-Jones, FNP-C.

April is Parkinson’s Awareness Month and we wanted to use this as an opportunity to share some of the early signs of the disease. No one sign means you have Parkinson’s. However, if you have more than one you should make an appointment to talk with your doctor. The earlier the diagnosis, the better as symptom progression can be delayed with exercise.

Thank you Redmond Chamber of Commerce & CVB for an amazing ribbon cutting last week! Our hearts are so full, and words cannot express our gratitude for this amazing community we get to serve. It's an honor and a privilege to do the work we do, and it means the world to us that our community supports our expanded mission. We are here for your friends and neighbors, always.

We were honored to celebrate ClearPath Healthcare with a ribbon cutting in Redmond.

Formerly known for their trusted legacy in end-of-life care as Hospice of Redmond, ClearPath Healthcare is stepping into a broader mission by expanding services to include specialized in-home primary care. Their new name reflects a continued commitment to clarity, continuity, and compassion for individuals and families across Central Oregon.

While the name has changed, the heart behind the care remains the same. We’re excited to see ClearPath Healthcare continue serving our community with the same deep compassion and trusted support, now with an even broader reach.

Congratulations to the ClearPath Healthcare team on this meaningful new chapter.
