If your pain has a pattern… pay attention.

Pain every month, during ovulation, or around your period is NOT normal — and it’s definitely not “in your head.” Cyclic pain anywhere in the body (pelvis, back, butt, legs, shoulder, abdomen) could be a sign of endometriosis.

Endometriosis is a whole-body, inflammatory disease — not just a reproductive problem — and early evaluation by an experienced specialist is critical.

✨ One day post-op and Carly already feels the difference!

Carly and Zach are heading back to LA today after her second endometriosis surgery with me. In 2020, just 3 months after her first surgery, she became pregnant naturally with their son Jack.

Nearly 6 years later, some symptoms returned—pain, bloating, and discomfort. She had surgery yesterday and already says she feels so much better. The bloating is gone, the pain is gone, and she’s starting to feel like herself again.

Endometriosis is chronic and can return, but with proper treatment, patients can reclaim their quality of life and fertility.

Carly first came to me in early 2020 with infertility, bloating, and pain after being referred by . We treated her endometriosis, and soon after, she welcomed her beautiful son Jack — with complete relief from symptoms.

Years later, after giving birth to her second child, Charlie, some symptoms started to return. Today, she and her husband Zach are back with us for her second surgery.

As we have been saying for decades, endometriosis is a whole body inflammatory disease. It can affect every organ of the body.

If you have butt pain, especially around your period, it could be endometriosis. Pain is not in your head!

MRI, ultrasound, and CT could all could be wrong!

You need to be investigated by an endometriosis specialist. If your physician is not familiar, refer them to Dr. Camaran Nezhat on Instagram!

Large paratubal cysts and endometriosis can sometimes contribute to pain and infertility by stretching, twisting, or distorting nearby reproductive structures. This patient’s cyst was repeatedly torsing and detorsing, causing severe, cyclic pain.

During surgery, we removed the cyst and restored the normal anatomy — a key step in improving both comfort and reproductive function. She has been trying to get pregnant, and after treatment, her prognosis is excellent.

Bilateral hydrosalpinges + severe endometriosis = IVF failure waiting to happen.

In many cases like this, IVF becomes a costly waste of time and money because the root problem is never addressed.

In this patient, both ovaries were severely scarred and “kissing” due to endometriosis. There was complete posterior obliteration, and both fallopian tubes were filled with fluid — a condition called hydrosalpinges — leaving them damaged and nonfunctional.

In the majority of similar cases, IVF success is extremely unlikely until the disease is treated.

That’s why we focus on excision surgery, clearing endometriosis and removing only what is truly damaged. In this case, the damaged tubes either have to be removed without causing further damage or tied.

I cut the bad and keep the good.

After treatment of endometriosis and fallopian tube removal, the prognosis is much better.

🔍 A rare find behind right-sided shoulder pain:
This patient’s symptoms led us to discover appendiceal intussusception — a condition where the appendix folds into the cecum, which can cause pain, obstruction, and other confusing symptoms.

On top of that, she also had endometriosis in the surrounding areas, making her pain even more complex and difficult to diagnose.

After excising the endometriosis and treating the intussusception, her condition improved significantly — and she is now trying to get pregnant without pain for the first time in years.

After an ectopic pregnancy in 2019, Ramya and her husband spent 6 long years trying to grow their family. They went from doctor to doctor, eventually turning to IVF — only to face two failed cycles.

When they finally met with Dr. Nezhat, he immediately recognized what had been missed all along: endometriosis. She underwent surgery in January 2024, and her prognosis afterward was excellent. Dr. Nezhat told them they even had a chance of conceiving naturally.

Six months later — just as he predicted — Ramya became pregnant without IVF. She went on to have a smooth pregnancy and a delivered a healthy baby girl. 💛✨

Gaslighting endometriosis is very easy because it can be completely asymptomatic.

Endometriosis is a whole-body disease — it can cause pain anywhere in the body. It can cause infertility without any signs or symptoms, organ loss (such as the kidney), chest pain, nosebleeds, belly button bleeding, butt pain, and more.

That is why we started — to teach physicians and allied health professionals about endometriosis and reduce the gaslighting and suffering patients experience. The average diagnosis delay is 7–10 years. This is not acceptable, and it must change.
