Most of my clients tell me they were never told how to turn and reposition for pressure injury prevention. Nor were they told what supplies and equipment to use for best results, and which to avoid. Most importantly, they were never told how and where to get more information.

So I wrote these discharge instructions for any caregiver to use as a template for providing written teaching for pressure injury prevention and management.

It is meant to complement the "How to Stop Bedsores" pamphlet, so that the discharging clinician can have a quick and efficient teaching tool that provides patient specific instructions and information. There is plenty of room to write in your own recommendations.

My experience through research, hospital work, and legal nurse consulting has shown me that when caregivers are required to turn and reposition patients strictly every two hours, the purpose behind the task can sometimes get lost—especially because caregivers have to consciously remember or are alerted to do it while they’re often busy helping a patient with something else. It can turn a meaningful intervention into a mechanical checkbox activity, which may unintentionally limit critical thinking and individualized care.
