As an internal medicine physician, I discuss how uncontrolled high blood pressure with fluid retention, such as swollen ankles and waking up short of breath, can be a sign of congestive heart failure. This condition can be exacerbated by testosterone replacement therapy, so I would not consider the patient an adequate candidate for hormone replacement until their blood pressure is under control and the fluid retention is addressed.

I explain that the increase in hemoglobin can occur in 20-25% of people receiving hormone replacement, and that not everyone who gets hormone replacement will develop steroid-induced acne. We discuss the need for blood removal due to high hemoglobin levels, and go over the potential complications of testosterone replacement therapy. Not everyone will experience these complications, but since I can't predict who will, I have all patients tested every three months. This allows me to identify those at greatest risk and have them undergo blood removal, so they can continue the treatment. The consultation usually lasts 40 minutes to an hour, and I provide a pamphlet to address common questions.

While the therapy may improve the quality of their sleep, it could also block their involuntary attempts to open their airway during sleep, potentially leading to a dangerous situation where they could die in their sleep.

Too many red blood cells can fill up the vascular compartment and slow down the delivery of oxygenated blood to tissues and organs. This can lead to stagnation and clot formation in certain areas of the body. To address this, regular blood checks are done, and if the levels are too high, blood removal (phlebotomy) is the standard treatment.

Reno Men’s Health: Mark Hahn, MD

Dr. Mark Hahn began Reno Men’s Health after an extensive career as a doctor of internal medicine with training in ICU critical care. As a doctor for the US Public Health Service, he treated the indigent population in rural California. Later, as a doctor for Kaiser Permanente in the San Fernando Valley near Los Angeles, he treated patients during the height of the AIDS epidemic. Throughout all of this experience, Dr. Hahn showed the utmost dedication to his patients and their care. When Dr. Hahn and his wife decided it was time to raise a family, they didn’t rest on their laurels. Instead, they opened and operated Orchard Urgent Care, where they relied on their extensive experience helping people to develop a caring, patient-first practice. After ten years running a successful urgent care center, Dr. Hahn’s dedication was recognized when he was asked to serve as the medical director of the Washoe County detention facility, where he oversaw the health and well-being of more than 1,100 inmates. In January of 2016, with the detention center’s health services improved and in good hands, Dr. Hahn narrowed his focus and began working with men’s health. For the past two years, he’s been helping the men of Reno, Nevada, by offering testosterone therapies and andropause relief. Today Dr. Hahn splits his time between the Reno Men’s Health practice, working with a renowned oncologist, and spending time with his family, with whom he lives in Reno.
