Our doctor is also very experienced in treating dry eye, a common condition in our Nevada climate. Dear Black Rock Vision Center Patients,
I am writing to inform you that the Visionary Eye Center, under my direction as Jason Bolenbaker, OD, has taken custody of the optometric patient records from the recently closed Black Rock Vision Center, the former practice of Dr. Michael Southam.
Please note that we are not purchasing the Black Rock Vision Center office or its assets. All financial matters, outstanding balances, glasses or contact lens orders or billing questions remain the sole responsibility of Black Rock Vision Center.

Your records are now in our possession and are available for continued care. Because many of the records are on paper and are currently being reorganized and stored remotely, we ask for your patience. Please allow up to 30 days for us to locate and process your specific file.

We are accepting new patients and would be delighted to welcome you to continue your eye care at the Visionary Eye Center.
