Are you a compassionate and dependable Licensed Practical Nurse (LPN) looking to make a difference in the lives of others? Join The Homestead Home Health and Hospice—a locally trusted, patient-centered agency dedicated to providing exceptional care and comfort to those we serve.

Position Summary
The LPN will provide skilled nursing care under the direction of an RN case manager, ensuring that each patient receives quality, individualized care in the comfort of their home or hospice setting. This role includes collaboration with a multidisciplinary team to promote physical and emotional well-being for patients and their families.

Key Responsibilities
• Deliver skilled nursing care in accordance with physician orders and agency policies
• Monitor and report patient conditions and progress
• Administer medications and treatments as prescribed
• Provide education and emotional support to patients and families
• Communicate effectively with RNs, physicians, and the interdisciplinary care team
• Maintain accurate, timely documentation of all care provided
Qualifications
• Current Idaho LPN license in good standing
• Prior home health or hospice experience preferred, but not required
• Strong communication, critical thinking, and documentation skills
• Reliable transportation and valid driver’s license
• Compassionate and professional bedside manner

Looking for an Activity Director for our Independent Living Cottages-

We’re looking for a friendly, organized, and energetic team member to help create a positive and engaging environment for our residents.

Key Responsibilities:

- Plan and facilitate daily activities that reflect residents’ interests and abilities.

- Drive residents to community outings and special events.

- Manage and track resident maintenance requests.

- Oversee the monthly activities budget to ensure meaningful and cost-effective programming.

- Encourage resident participation and engagement in daily activities.

- Assist with serving lunch in both the cottages and assisted living buildings.

- Act as a communication link between residents and the facility manager, ensuring needs and feedback are addressed promptly.

- May also help Homestead management with some clerical, administrative, and organizational tasks

Ideal Candidate:
You’re compassionate, creative, and enjoy bringing joy and connection to others. You have strong organizational skills, a positive attitude, and thrive in a team-oriented environment.

Hours are Mon-Fri 9:30am-3pm. Some activities will require you to be flexible with hours.

Pay starts at $12/hr
