Most people think low back pain comes from something being “tight.”

But one of the most overlooked contributors is a deep spinal stabilizer called the multifidus.

The multifidus sits along your spine and plays a key role in:
• Controlling movement between each vertebra
• Providing segmental stability
• Helping your body handle load efficiently

Here’s what’s important 👇
After injury—or even just ongoing pain—the multifidus can shut down or become inhibited.

And it doesn’t always turn back on automatically.

When that happens:
• Larger muscles try to compensate
• Movement becomes less controlled
• Load shifts to structures that shouldn’t be taking it
• Pain can persist or keep coming back

That “tight” feeling people describe is often protective guarding of pathological tissue, not a true flexibility issue.

In this image, we’re targeting the multifidus using dry needling with electrical stimulation to:
• Improve blood flow and activation
• Restore timing and coordination
• Reduce guarding and pain
• Re-establish spinal stability

This is one piece of the puzzle—but it’s often a missing one.

This is something we see all the time with runners—from beginners to high-level athletes.

And here’s the reality…

👉 It’s usually not your knee.

In our experience working with runners, knee pain is most often a result of how you’re moving, not just where you feel it.
