African Americans continue to face higher rates of chronic and cardiovascular conditions, often compounded by delayed diagnosis and limited access to ongoing primary care. Heart disease remains the leading cause of death in the U.S., making proactive management essential to preventing downstream complications and costly ER utilization.

The data underscores the urgency:

● African Americans experience higher rates of high blood pressure than Caucasians (Ages 18–34: 12% vs. 10% | Ages 35–49: 33% vs. 22%)

● Diabetes is more prevalent among African Americans ages 35–49 (10% vs. 6% among Caucasians)

● Stroke risk is consistently higher across age groups (Ages 18–34: 0.7% vs. 0.4% | Ages 35–49: 2% vs. 1% | Ages 50–64: 7% vs. 4%)
