The Pediatric Symptom Checklist-17 (PSC-17) is a quick, research-based tool that helps parents notice areas where their child may need extra support.

Read each question and check the box that best describes your child: Never, Sometimes, or Often.
If you find yourself checking more boxes on the right-hand side (Sometimes/Often), it may be time to talk with your child’s doctor, teacher, or a mental health professional.
