When mammogram screenings became widespread in the late 1980s, something remarkable happened. For the first time in 50 years, the breast cancer death rate began to fall and it has kept dropping ever since. More than 600,000 lives have been saved and that number keeps growing. Early detection works.

The diagnostic workup revealed what the initial screening hinted at. But here’s the question: why was this cancer missed in 2019?

Findings:
◼️ Diagnostic mammogram: focal asymmetry with architectural distortion persists
◼️ Ultrasound: irregular heterogeneous mass with indistinct margins at 12 o’clock, 5 cm from the ni**le
◼️ Mass was palpable on physical exam
◼️ Pathology: confirmed malignancy

Spoiler: comparing only to a recent prior exam was part of the issue. Want the full story?

A 56-year-old patient came in for routine screening, but something caught the reader’s eye in the 2023 images. Would you call her back?

Findings:
◼️ 2023 screening mammogram: focal asymmetry with associated architectural distortion in the superior central breast
◼️ 2019 screening: similar finding, but not corresponding to prior surgical excision site
◼️ 2011 screening: no abnormality in that area

This case highlights how subtle findings can evolve over time. Ready to sharpen your skills?
