Great advice to get your records. Especially if you’ve had a recent ER visit, hospital stay or plan on joining our practice!

DCHD advises you to request your Crozer medical records as soon as possible, because Prospect has not responded to inquiries about how long the below resources will be available.

There are 2 main ways to get copies of your medical records (both are FREE, you will NOT be prompted to enter any type of payment information):

1. Via the Medical Records Request form on Crozer Health's website: https://www.crozerhealth.org/patients-visitors/medical-records-request/

2. Via the HealthLink patient portal: https://pmh.consumeridp.us-1.healtheintent.com/saml2/sso/login?authenticationRequestId=1f3a827e-d12e-49a3-9033-489ab44de584

Reminder for ESTABLISHED patients! Please use our website, www.internassoc.com for an extremely quick and easy way to book appointments.

With as easy as 2 clicks, (see pictures below), you’ll be able to select between our four providers and pick a date and time you want as the link takes you to our Healow app booking site.

We encourage this as we navigate the increase in phone calls. We are accepting new patients. We are staying in Ridley park like we have been since the 1960s! Thank you again for your patience. Please spread the word.

Do not panic! Regardless of what happens to our neighbors owned by Prospect Medical Holdings (Crozer, Taylor Hospital, ect), we will remain OPEN. We are not owned by them. Rest assured, we will continue to help the community.
