We are a home based model offering individualized care for our residents. We care for seniors with dementia from mid stage through end of life. Autumn Hills is our residents' home and our goal is to promote their happiness and health. We have only licensed and/or certified nursing staff that care for our residents 24/7. We have a 1:4 resident to staff ratio which allows us to provide the best care

to our residents. Many of our staff are AFA Qualified Dementia Care Providers. Our facility is beautiful and was built for the purpose of caring for seniors. You can take a virtual tour and learn more about us on our website.
