🩺 Understanding Chronic Conditions: What We Manage in Primary Care

Did you know that managing chronic conditions is one of the most important things we do at your primary care office?

In this short clip, Dr. Tabrez discusses the wide range of long-term health issues we help Brandon and Riverview residents manage every day.

From the heart to your lungs and overall wellness, primary care is your home base for comprehensive health management.

Conditions we manage include:

- Cardiovascular: Hypertension (high blood pressure), Hyperlipidemia (high cholesterol), and Coronary Artery Disease.
- Respiratory: Asthma, COPD, and follow-up care for conditions like Lung Cancer.
- Metabolic: Obesity, Diabetes, and Osteopenia/Osteoporosis.
- Mental Health: Depression and other mental health concerns that require ongoing management.

Effective management is key to a healthier, happier life.
Don't wait...regular check-ups allow us to monitor your health and adjust your care plan as needed.

☕ "But Coffee Is Mostly Water, Right?" ☕

We hate to be the bearer of bad news, but... your 3 cups of coffee are NOT hydrating you the way you think they are.

Here's why:
Yes, coffee contains water.

BUT caffeine is a diuretic - meaning it makes your body produce MORE urine and lose fluids. So while you're taking in liquid, you're also pushing it right back out (often more than you took in!).

Think of it this way:

Drink a cup of coffee → Your body uses some of that water
BUT the caffeine signals your kidneys to flush out MORE water than the coffee provided

Net result? You're actually LOSING hydration

What DOES count as hydration:
✅ Plain water (the gold standard!)
✅ Herbal tea (no caffeine)
✅ Water with lemon, cucumber, or fruit
✅ Sparkling water (unsweetened)
✅ Coconut water
✅ Most of the water content in fruits and veggies

What DOESN'T count (or counts less):
❌ Coffee
❌ Regular tea (has caffeine)
❌ Energy drinks
❌ Soda (also dehydrating!)
❌ Alcohol (very dehydrating)

Does this mean you have to give up coffee?

ABSOLUTELY NOT! ☕ We're not monsters! 😄

Just don't count it toward your daily water goal.

When you hear "chronic disease" - diabetes, high blood pressure, heart disease - what do you think causes it?
Most people say: bad diet, smoking, not exercising.

𝐀𝐧𝐝 𝐲𝐞𝐬, 𝐥𝐢𝐟𝐞𝐬𝐭𝐲𝐥𝐞 𝐦𝐚𝐭𝐭𝐞𝐫𝐬. 𝐁𝐮𝐭 𝐭𝐡𝐚𝐭'𝐬 𝐍𝐎𝐓 𝐭𝐡𝐞 𝐰𝐡𝐨𝐥𝐞 𝐬𝐭𝐨𝐫𝐲.

Here's what we need to talk about more:

𝐀𝐜𝐜𝐞𝐬𝐬 𝐭𝐨 𝐡𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞.

If someone can't afford to see a doctor for annual check-ups...
If they can't get their screenings done...
If labs are too expensive...

𝐓𝐡𝐞𝐲 𝐞𝐧𝐝 𝐮𝐩 𝐰𝐢𝐭𝐡 𝐜𝐡𝐫𝐨𝐧𝐢𝐜 𝐜𝐨𝐧𝐝𝐢𝐭𝐢𝐨𝐧𝐬 𝐭𝐡𝐞𝐲 𝐝𝐢𝐝𝐧'𝐭 𝐞𝐯𝐞𝐧 𝐤𝐧𝐨𝐰 𝐭𝐡𝐞𝐲 𝐡𝐚𝐝.

Not because they didn't care about their health. Not because they made bad choices. But because they couldn't access preventive care when it mattered most.

𝐂𝐡𝐫𝐨𝐧𝐢𝐜 𝐜𝐨𝐧𝐝𝐢𝐭𝐢𝐨𝐧𝐬 𝐚𝐫𝐞𝐧'𝐭 𝐣𝐮𝐬𝐭 𝐚𝐛𝐨𝐮𝐭 𝐟𝐨𝐨𝐝, 𝐬𝐦𝐨𝐤𝐢𝐧𝐠, 𝐚𝐧𝐝 𝐚𝐥𝐜𝐨𝐡𝐨𝐥. 𝐓𝐡𝐞 𝐡𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞 𝐬𝐲𝐬𝐭𝐞𝐦 𝐩𝐥𝐚𝐲𝐬 𝐚 𝐇𝐔𝐆𝐄 𝐫𝐨𝐥𝐞.

Someone could be doing everything "right" - but if they can't afford the blood work to catch prediabetes early, or the blood pressure check that would show hypertension, they don't get the chance to prevent it from becoming a serious problem.

This is exactly why preventive care matters so much. And why we work hard to make care accessible - payment plans, working with insurance, finding generic alternatives, connecting people to resources.

Here's what you can do:
✓ Prioritize your annual check-up when you CAN access care
✓ Ask us about cost concerns BEFORE skipping something - we can often find solutions
✓ Know what screenings you need and advocate for yourself
✓ Don't wait until you're really sick to come in

Healthcare access matters just as much as lifestyle choices. Both are part of the picture.

What's one barrier that's kept you from getting healthcare when you needed it?
Let's talk about it - no judgment, just honesty. Maybe we can help problem-solve together. 💙

𝐂𝐚𝐧 𝐖𝐞 𝐓𝐚𝐥𝐤 𝐀𝐛𝐨𝐮𝐭 𝐭𝐡𝐞 𝐒𝐮𝐩𝐩𝐥𝐞𝐦𝐞𝐧𝐭 𝐀𝐢𝐬𝐥𝐞? 💊

You walk into the pharmacy or health store and there are literally HUNDREDS of supplements promising everything from better sleep to stronger immunity to clearer skin.

Most people with a reasonably balanced diet don't need most supplements. BUT some people DO need specific ones, and that's where testing comes in.

𝐈𝐧𝐬𝐭𝐞𝐚𝐝 𝐨𝐟 𝐠𝐮𝐞𝐬𝐬𝐢𝐧𝐠, 𝐥𝐞𝐭'𝐬 𝐭𝐞𝐬𝐭:
- Vitamin D levels (very common deficiency, especially in less sunny months)
- Iron/ferritin (if you're always tired or have heavy periods)
- B12 (especially if you're vegetarian/vegan or over 50)
- Thyroid function (affects energy, weight, mood)

𝐓𝐡𝐞 𝐌𝐨𝐬𝐭 𝐈𝐦𝐩𝐨𝐫𝐭𝐚𝐧𝐭 𝐐𝐮𝐞𝐬𝐭𝐢𝐨𝐧 𝐈 𝐀𝐬𝐤 𝐄𝐯𝐞𝐫𝐲 𝐏𝐚𝐭𝐢𝐞𝐧𝐭 🤔

It's not about your symptoms. It's not about your medications.

𝐈𝐭'𝐬: "𝐇𝐨𝐰 𝐚𝐫𝐞 𝐲𝐨𝐮 𝐝𝐨𝐢𝐧𝐠... 𝐫𝐞𝐚𝐥𝐥𝐲?"

Because here's what I've learned: when people say "I'm fine" while making that face 😬... they're usually NOT fine.

Sometimes what's affecting your health the most isn't a virus or a lab number - it's:
- The stress you're carrying
- The sleep you're not getting
- The grief you're processing
- The anxiety you're managing
- The life transition you're navigating

Your mental and emotional health directly impacts your physical health. They're not separate departments - they're one system.

So when I ask "how are you really doing?" - I genuinely want to know. Not just about your blood pressure, but about YOU.

𝐇𝐞𝐫𝐞'𝐬 𝐦𝐲 𝐢𝐧𝐯𝐢𝐭𝐚𝐭𝐢𝐨𝐧 𝐭𝐨 𝐲𝐨𝐮: Next time someone asks how you're doing, try giving a real answer. Not to everyone - but to someone safe. Your doctor, a friend, a therapist, someone.

𝐋𝐞𝐭'𝐬 𝐬𝐞𝐭𝐭𝐥𝐞 𝐭𝐡𝐢𝐬 𝐨𝐧𝐜𝐞 𝐚𝐧𝐝 𝐟𝐨𝐫 𝐚𝐥𝐥!

Are you a...?

🌅 𝐌𝐎𝐑𝐍𝐈𝐍𝐆 𝐏𝐄𝐑𝐒𝐎𝐍 - "I'm up before my alarm, ready to conquer the day!"
🌙 𝐍𝐈𝐆𝐇𝐓 𝐎𝐖𝐋 - "My brain doesn't turn on until after 10am, but I'm unstoppable at midnight!"

𝐂𝐨𝐦𝐦𝐞𝐧𝐭 𝐲𝐨𝐮𝐫 𝐭𝐲𝐩𝐞 𝐛𝐞𝐥𝐨𝐰! And be honest - not what you WISH you were, but what you ACTUALLY are! 😄

𝐃𝐨𝐜𝐭𝐨𝐫'𝐬 𝐧𝐨𝐭𝐞: There's actually science behind this! Your chronotype (natural sleep-wake preference) is partly genetic. Neither is "better" - but fighting your natural rhythm can affect your health. The key is getting quality sleep, whenever that happens for you.

Bonus question: If you could redesign your schedule around YOUR ideal sleep time, what would it look like? 💭

Compassionate, Convenient and Quality Medical Care

Coming from a family with so much background in healthcare, I knew I would always pursue this path.

I saw that the healthcare system had become difficult for patients to navigate...making it even harder to understand their health needs. So the idea was to provide a patient-centered home that was easy to access and use.

Everyday is exciting and brings new challenges but also quite rewarding knowing that people always have a place to come for their healthcare needs.

Over the last 8 years, I’ve been able to build ties with local hospitals and other specialists to better serve our community.
