Old Southwest staff wore blue today in support of Autism Awareness Month in honor of Cam, our Business Office Manager’s son. 💙

We also enjoyed a visit from the Hangry food truck, bringing great food and a great time for everyone. It was a meaningful day of support, awareness, and community togetherness.

Our dedicated staff received their certificates after completing specialized training focused on improving care, strengthening education, and enhancing support for our residents living with heart failure. ❤️

Meet Troy, our April success story at Old Southwest Health and Rehab! 🎉

After spending several months with us, we are excited to celebrate Troy as he heads home today stronger and ready for his next chapter.

Troy shared that his experience was very positive, highlighting how “everyone here is pleasant” and how physical therapy, exercise, and walking helped him regain strength and improve his health during his stay.

We are so proud of your hard work and determination, Troy! You are a true example that with commitment and effort, great progress is possible. Wishing you all the best at home, and we can’t wait for your visit back! ❤️

At Old Southwest Health and Rehab we had an incredible turnout for our Heart Failure Program outreach event last week!

We prepared close to 200 goodie bags, and in less than an hour, every single one was handed out! We were so grateful to connect with our community while sharing helpful resources, booth swag, and our Heart Failure brochures 🙂

Thank you to everyone who stopped by and helped make this event such a success!

Our Heart Failure Program at Old Southwest is supported by a collaborative group of professionals, including Admissions, MDS, Nursing, Social Services, and our Nurse Practitioner. Together, they provide coordinated care, education, and support to help residents manage heart failure safely and effectively.

Teamwork truly makes the difference in patient outcomes.

You may remember Robin as our June 2025 success story here at Old Southwest. After a recent hospitalization in March, she returned to us for therapy and nursing services, and once again, she has made tremendous progress.

When asked why she keeps coming back to Old Southwest with so many choices out there, Robin shared, “Y’all working with me and stuff like that.” That kind of trust means everything to our team.

Robin is set to return home tomorrow, and we couldn’t be more proud of her hard work and determination. We wish her the very best and are so grateful she continues to trust us with her care. Safe travels home, Robin, we’ll be cheering you on every step of the way! 👋
