If needing a school or sports physical call our office, $40 for either one. We do take walk in but prefer appointments due to waiting time. Call our office 815-227-9594. Our provider is in Rockford on Mondays, Wednesdays, Thursdays and Fridays, and in Rochelle on Tuesdays and Thursdays. Saturdays various so contact our office.
