▪️Do you use the Strategic Assessment of Risk and Risk Tolerance (StARRT) framework to aid in return to sport decision-making? What about the version for returning to run following injury that I just made up by changing a few words?

▪️Actually though, it does help me to see a (non-exhaustive) list to make sure I’m considering as many factors as possible. Practically, I grade them with a traffic light scale because we can’t assign a precise weight to any of these variables in a given case.

▪️The risk assessment process involves factors specific to the injury and the activity. After that, contextual modifiers of the situation that don’t otherwise apply to the other steps are used to determine how much risk we can allow when returning someone back to sport (or running… I forgot where I was going with this).

The show will be an interview-style podcast focused on highlighting current evidence in sports PT and the career paths of professionals in the sports world. I will be hosting the monthly episodes geared towards research and clinical practice, and Scott hosts the episodes learning from professionals.

Buchheit & Lauersen 2013 two-part review on high-intensity interval training.

Some really nice, practical information on manipulating variables like rep duration, intensity, rest, and volume to target your goal. Great references to have for designing both endurance and team-sport conditioning.

▪️Quadriceps eccentric overload 🦵🦵🦵

▪️The quads are the breaks - they decelerate and overcome significant breaking forces. Training them eccentrically builds the capacity to help do this.

▪️Not as accessible as the Nordic hamstring exercise (which doesn’t require a machine or external load for most individuals) but a pretty simple* way to achieve true eccentric overload. Typically I’ll have someone ramp up a few sets single leg concentrically until they get to a weight that they can’t push past 45° for a single rep, then the fun starts. was probably going a little too light here, since there’s no immediate lag once he removes his second leg.

*do not mistake simple for easy

▪️3-4 sets of 3-5 reps is my starting point here, and I don’t plan to do anything important for the next two days if it’s the first time doing this in a cycle 😂

💪Every sport has its own equipment, for runners, it's all about footwear! Growing up we used the “fast shoe” test in the store! Lace it up, run as fast as you can around the store, if you felt fast this was the shoe for you. As more and more styles of running shoes have become available the question that needs answering is which shoe should we be using?

✅This question is answered and a lot of myths are busted in this podcast as Dillon discusses with Jay all topics related to running shoes. They discuss the barefoot running craze and the explosion of minimalist running shoes after the 2009 book Born To Run was released to the other end of the spectrum, maximalist shoes, and everything in between. 

👟Benefits of minimalist shoes: 
➡️Lighter shoes can improve running economy, however, barefoot running can cost more from a metabolic standpoint due to increased muscle demand. 
➡️Variability in footwear, when used intelligently, can be a great training tool. 


‼️ Important Reminders:  
➡️Drastic changes in running shoes cause an increase in workload to different joints even if you keep the same running volume and intensity! 
➡️Minimalist shoes bring more force to the ankle/foot. Make sure these areas are ready for the force increase if switching.  
➡️Footwear is not the main factor in injury reduction for runners.
➡️Natural fallacy: the idea that what is found in nature is good, which sometimes is true! Other times we have to respect the advancement of technology that has made tools for us to better perform in our environment. Example: The best running times have been set by athletes using running shoes with high-stack carbon fiber plate shoes. 
➡️We don’t have strong evidence that pronation is an important injury risk factor. Bringing up the question, do we really need to match the foot to the shoe or just use the comfort paradigm or fast shoe test?

Nice review recognizing that the S&C literature is largely on uninjured athletes, but major principles like the needs analysis and program design apply in the rehabilitation / restoration of load capacity of the injured athlete.
