TruCare Connections is a Home Care Services and Health Homes Care Management Agency based in Rochest

Welcome to our Consumer Directed Personal Assistance Program (CDPAP). The Consumer Directed Personal Assistance Program is a Medicaid funded home care program in New York State. The program allows people with disabilities and/or personal care needs to have more control over their personal assistance services. Our program enables individuals to directly affect their quality and continuity of home care services. The self-directed consumer is responsible for the recruiting, interviewing, hiring, training, scheduling, supervising, and terminating Personal Assistant(s) of their choice. The event was vibrant and full of fun activities, and the food was absolutely amazing. A big shoutout to our incredible Party Planning Team in Syracuse for their brilliant work in making this celebration possible!

A heartfelt thank you to all our staff who made it to the celebration—you truly made the day special.

This year’s gathering felt a little incomplete without our CDPAP Department staff, but we’re looking forward to an amazing year ahead together.
