RAWNY Image Feature of the Month: Hypocyanescent lesions on Ultra-widefield Indocyanine Green Angiography (ICGA). Hypocyanescent lesions are dark spots observed in the choroid during Indocyanine Green Angiography (ICGA) that indicate reduced perfusion, underlying inflammation, or fluid blockage. They are key diagnostic markers for ocular inflammatory conditions like birdshot chorioretinopathy, Vogt-Koyanagi-Harada disease, and uveitis, often identifying reduced choroidal filling not visible on standard dilated exams. This patient has been diagnosed with multifocal choroiditis, which is a form of uveitis. Other interesting features on this image are the distinct vortex veins only visible on ICGA, which are large peripheral vessels responsible for the venous drainage of the choroid.
