In recognition of National Apprenticeship Week 2026, the Community Health Workers Association of Rochester (CHWAR) proudly acknowledges the continued impact of the Ibero-American Action League’s Community Health Worker (CHW) Registered Apprenticeship Program. As Black Maternal Health Week concludes, we return to what the data demands: prevention through equity.

NYS reporting (2018–2020) shows pregnancy-related deaths of 54.7% for Black, non Hispanic women vs 11.2% for White non Hispanic women, and found 73.6% of pregnancy-related deaths had some chance of being prevented. In the Finger Lakes maternal mortality is 60% higher than the NY State average. This is not acceptable.

Reducing preventable harm requires sustained action, including: respectful and culturally-aligned maternity care practices, community-based support models, and systemic investments that reduce barriers to care across the lifespan.

The good news is that many partners are already coming together to address maternal health disparities. From the Healthy Baby Network to HealthConnect One to officials from the City of Rochester & Monroe County, NY, Rochester Regional--and many, many more--we are working to pair lived experience with quantitative data to better understand the challenges and provide the services that Black women and families need.

Together, we can make sure that every birth is safe. That all care is equal. And that pregnancy and childbirth are reasons for celebration, for every woman and family in our region.

Our 2025 Year in Review is here.
In 2025, Common Ground Health and the Rochester RHIO turned data into action across Rochester and the Finger Lakes—advancing health equity through community partnerships, practical tools, and trusted information sharing.

A few highlights from the year:

➤ Love from the Start: 700+ ads delivered more than 17 million impressions promoting the importance of early relationships for infants and toddlers.
➤ Community overdose prevention: 6,869 Narcan kits distributed, and trusted messengers helped connect hundreds of people to health resources.
➤ Rochester RHIO reliability and scale: 1.7 million residents served across 14 counties, with 99.89% network uptime, plus 28 million Results Delivery messages and 1.2 million alert messages supporting care.

This new year as a combined entity with Common Ground Health, Finger Lakes Performing Provider System - FLPPS, and Rochester RHIO, we're poised for even greater impact.
