Staffing is very limited during weekend shifts and we regret that we will not be able to refill medications, fill out camp forms or process any referrals. Request for appointments for well child care should be scheduled during regular business hours with your providers receptionist. Pen Bay Pediatrics uses a medical home model in our patient care. A "Medical Home" is a model of patient and family

centered, community-based care that provides continuity of care from childhood through adolescence. It was developed to deliver primary care that is accessible, compassionate, and culturally effective. This approach creates a comprehensive and collaborative working relationship between numerous clinicians, their patients, and the patients' families. (Adapted from NICHQ)
