PMR
Stands for polymyalgia rheumatica .
I’ve been seeing a lot of this lately and I can’t say why. But I think it’s worth talking about. This diagnosis tends to be most common in men and women between the ages of late 50s to late 70s.  in most cases it starts quite insidiously and confuseing with pain in the shoulders and hips/pelvis sometimes shoulders first sometimes hips first …in the early morning and improving during the day.
It keeps getting worse with time  and can become quite miserable. 
The bad news is many primary care physicians don’t see this very often and can take quite a bit of time to make the diagnosis. The good news is rheumatologist see this every day, and the treatment is effective and swift.  Left untreatment the condition can progress and become serious. 
 Short piece of advice: if you are in your late 50s into your 80s and something is just not right and you’re not feeling well and you have pain particularly in the areas I just described ask your doctor to check for inflammation (ESR, CRP) and if out of range a referral to rheumatologist might be the best thing.

Happy New Year!

This year brings many exciting new diagnostics and treatments for rheumatic diseases. I’m more excited than ever to treat these complex and chronic conditions with more success than ever before.

Sadly my office will no longer be offering treatment to those insured by Medicare advantage plans. We have had a terrible year with denial of care, long intensive battles to acquire prior authorizations for treatments easily covered by traditional Medicare and then denied again even after granting prior authorization.
If you have been a patient in my practice we will provide all your records and are available to speak with your new Rheumatologist if requested.

We are looking forward to helping our patients with osteoporosis, arthritis and rheumatic conditions this year with all the new and exciting modalities available in 2025.

THE ANA TEST

I’ve been asked to consult on many patients found to have
a “positive ANA test” This is a typical type of consult for any Rheumatologist but I’ve noticed more lately (don’t know why) and have noticed a great deal of patient anxiety about this.
So let’s get to the basics:
First of all the test is somewhat over used to screen for autoimmune disorders. When the result is negative the Lupus related Syndromes are highly unlikely. Unfortunately when it’s positive it is very often a false positive … meaning the test is either inaccurate or it is accurate but means nothing abnormal.
It’s good to get it checked out by a board certified Rheumatologist but don’t fret before your appointment. More often than not it’s not indicative of an autoimmune diagnosis. And of course if it is… Rheumatologist know what to do 😉
