In fertility studies, women with poor ovarian reserve who took CoQ10 at a dose of 600 mg per day (usually 200 mg three times daily) for 8–12 weeks before IVF stimulation showed improved ovarian response, including a higher number of retrieved oocytes and better embryo quality compared to controls.

The proposed mechanism is improved mitochondrial energy production in the oocytes, which may enhance egg competence.

However, while egg numbers and response to stimulation may improve, research has not consistently shown a significant increase in live birth rates, and CoQ10 does not reverse age-related chromosomal abnormalities.

The 600 mg dose is generally well tolerated, with mild side effects such as stomach upset or headache in some individuals.
