"There are two types of death certificates issued by the Center for Health Statistics: the short form (without cause of death), and the long form (with cause of death). The fee for both types is $25.
Short Form Death Certificates (without cause of death)
This certificate includes the name of the deceased, the date of the death, the location of death, as well as personal information about the deceased like their address, birth date, name of parents and spouse/partner (if applicable). It does not list the cause of death, time of death, or any medical information. This certificate provides proof of the fact of death when the cause and manner of death are not needed.
