🌿 This month, TransitionPLUS participants have been busy—and having fun! We enjoyed a trip to the American Dream Mall, taken in the breathtaking views at Eagle Rock Reservation in West Orange, and continued tending to our community garden.
What is TransitionPLUS?
It’s a program designed for individuals with disabilities ages 21–25+ to build independence through daily living and pre-vocational skills. With structured support, participants gain tools to navigate life’s challenges, strengthen self-advocacy, and prepare for a future full of possibilities.
Our program offers:
✅ Individual assessments & goal setting
✅ Independent living skills training
✅ Career exploration & resume building
✅ Volunteer experiences & community integration
✅ Leisure activities & social skill development
✅ Case management & peer networking
Participants attend at least 2 days per week, Monday–Friday, 9 AM–3 PM. Eligibility is through the NJ Division of Developmental Disabilities (DDD).
