Not ruptured… but not normal either.

These implants were removed en bloc, meaning the implant and capsule were taken out completely intact. On the surface, everything may appear “fine”—but a closer look tells a different story.

Both implants showed visible creasing, which is where the shell weakens over time. This is often how leaks begin—not always with a dramatic rupture, but with subtle structural changes that you can’t feel or see from the outside.

Even more interesting—
The capsule surrounding the implants was extremely thin, almost transparent, yet still strong enough to be removed intact.

These implants were still intact—but that doesn’t mean everything was okay.

In this case, one implant showed a deep crease, indicating it was likely on the verge of rupture, while both were affected by significant capsular contracture.

When scar tissue tightens around the implant, it can distort shape, increase pressure, and weaken the integrity of the implant over time.

👉 This is how problems can develop before an actual rupture occurs.

As shown here, both implants were removed intact during en bloc removal—ensuring the surrounding capsule and any affected tissue were taken out together.

The takeaway:
Just because an implant hasn’t ruptured doesn’t mean it’s healthy.

“Not all implant failures look the same.”

This case demonstrates two different findings:

• One implant with a true rupture (tear)
• The other showing early silicone leakage (gel bleed)
• Both contained within an extremely thin capsule

From the outside, these can appear identical.

But internally, the difference is significant.

By performing a true en bloc capsulectomy, both implants were removed intact within the capsule—helping to:

✔️ Contain silicone
✔️ Prevent spread into surrounding tissue
✔️ Maintain control throughout the procedure

⚠️ Important for patients to understand:

An implant does not need to be fully ruptured to be compromised.

And in many cases, there are no obvious symptoms.

This is why experience and surgical technique matter when it comes to safe, complete implant removal.

So happy with today’s surgery. I removed 38 year old, ruptured silicone implants via enbloc capsulectomy. By performing enbloc, I was able to prevent any silicone from migrating into her surrounding tissue. In the photo, I opened up one capsule to show the leaking silicone. All of the ruptured silicone was contained within the capsule, demonstrating the importance of enbloc, removing the capsule and implant in one piece. Both capsules had hard, capsular contracture. A breast lift was also performed, giving her a much more youthful appearance.
