Massive PE is when someone is about to die from a massive lung clot where blood from the right side cannot get to the left. Often if patient actually survives and make it to the hospital alive, then are given a high dose of a clot busing medication called TPA. However, that medication can cause serious bleeding, especially into the brain! Here we use a new device that allow us quickly to restore blood flow to the lungs and hence the left side of the heart. Luckily everything went well and we were able to save the man's life who is going home on no oxygen and no major morbidity.

“60 y/o male with ALS, passed out the minute he woke up! In ED, SBP 82 mmHg. Echo in Resusc with blown-out RV -> Fluids and dopamine. CT with obstruction of both PA. CT to device time: 15 minutes. SBP 120 after thrombectomy. 1st massive rxd that way ”

Answer:

The use of hydroxychloroquine + Azithromycin is currently based on some anecdotal data in vitro and NOT based on major trials. We have to be careful because combining the two meds has the potential to increase the QTC and lead to malignant ventricular arrhythmias. My advice is to NEVER take a med unless it's prescribed by a specialist. All meds are poisons...the reason why we prescribe them to you is that the Benefit often >>>>>risks of side effects. All meds have the potential to hurt you...Some meds, especially in combination, require careful monitoring.

Hence, I am not against the use of said medications per se as we literally have No data regarding the best way of treating COVID-19. I am in favor of studying them in RCT as we go along (along with other therapies as Dr. Fauci has outlined today).

We sometimes have to use "biological plausibility" in approaching new therapies until data materializes...And there is evidence that Chloroquine does disrupt virus proliferation in addition to the interesting "observation" of low COVID-19 incidence in countries endemic with malaria due to "presumably" increased use of Chloroquine. Only time will tell what works and what doesn't.
