Combining the private practice implant knowledge with head and neck hobby on this one.
Restoring form and function so that no one can tell the patient lost half their jaw to the tumor.

We give everything we have to take care of head and neck cancer patients primarily to make them better and cancer free.
However when I talk to residents and students the highlight of the conversation is mostly science and we don’t always have the opportunity to discuss things like patient centered approach to care and addressing social issues. As I type this I think that it’s also so beyond an IG post.
So to be back in the science of it, head and neck anatomy is like no other! Comment some of the structures you can name here

I want to thank all my followers and friends for the overwhelming support of my new page over the past 24 hours. It's been super exciting seeing everyone follow the account. I want to clarify that we are SUPER early in the process and the entire project of building the surgical center is ahead.
On another note, I am still at and and operating in full swing. I just want to say, even though it has not happened yet, it's truly bitter sweet. It has been amazing 3 years working there and I'm super grateful to my surgical partners and the staff there for mentoring me, teaching me and just being my friends this whole time (and the prior almost decade with since first day as interns in ).
I have zero regrets joining this practice as my first out of training. I am grateful for every day I spend (and still do) here and every opportunity I got to work with these amazing talented surgeons. Dr. Hoffman has been an amazing mentor and a role model. I still can't get over this man’s talents, ingenuity and love for his craft. Dr.Lam is a total badass of a surgeon and a true implant queen (yes that's what she is around here). I will forever be there for you anytime you ever need me.
All love, lots of excitement (with a little bit of nervousness) and as my said, Lets F$%ing GO!!!

Sometimes large asymmetries call for deviation from usual.
With help of virtual surgical planning we’re able to foresee potential road bumps and correct it early on.
Failing to plan is planning to fail.
In this case we performed a mandibular advancement via inverted L osteotomy with rigid fixation transorally on the left side. The defect is grafted with allograft and BMP and autogenous bone.
Right side was set back via conventional sagittal split osteotomy. Single piece Lefort I and genioplasty. All with patients specific fixation.

Implant case once again. I really do love putting a smile on my patients faces. .cvg definitely makes my top 🔝 10 favorite patients list
This is Cristel and she’s on her journey to becoming a pediatric dentist. 🤛🏼👩🏼‍🔬
Cristel initially came to me to replace a missing tooth 🦷. It wasn’t an easy case to say the least. Imagine a young pretty patient who is a future dentist with a paper thin maxilla 😱. Yes it needed to be perfect.
I initially grafted the ridge and while in recovery room she got so upset at me, to a point of tears. To be honest i wondered what was wrong but then found out that she wanted to have her surgery live on Instagram. Even though we trained in the same place I’m no 🤣
Well now we’re here. Bone healed well and we were able to get in an implant with of course palatal roll soft tissue graft and immediate temp crown.
Can’t wait to have the braces off and go to a permanent restoration.
Esthetic implant but also head and neck surgeon 👨🏻‍⚕️ 🤪 My residents ask me if I get bored doing dentoalveolar surgery, especially in the OR during head and neck cases, when they’re in awe. As awesome as it is to do neck dissections and parotidectomies at the same time curing cancer, putting a smile on Cristels face couldn’t make me happier.

Since mentioned marginal mandibular nerve, here is the dissection
We usually don’t particularly “look” for it but rather just protect it. In some cases it jumps right at you and in others you never see it. Bottom line is its anatomy is variable and the guidelines summarized in post help us protect it
Here is a case of submandibular gland carcinoma, which is known to have perineural spread. Marginal nerve is deliberately dissected. Does it need to be sacrificed? Comment below with your answers
Note the “pre dissected” neck appearance with no structures identified and neck full of lympho-adipose tissue
Second photo showing the proverbial “gateway” into the neck, the platysmal flaps with superficial layer of deep cervical aka the investing fascia looking at you
Last picture is post dissection. Note the spinal accessory nerve and it’s caliber as a comparison to the marginal mandibular nerve
This was one of the first cases early in my practice. Good catch by my dental colleagues. Adenoid cystic ca is slowly progressing but deadly disease. This patient had VERY early catch 🤞🏼

If quarantine can NOT stop people from getting hurt, or get infections, toothaches and cancers, the COVID outbreak 😷 can NOT stop our from helping our patients.
Even though it’s been a tough block, our current resident (who’s following in one of our owns footsteps, and pursuing a subspecialization on Head & Neck Oncologic surgery) has done a fantastic job . 🔩⚙️🔧🔨

🦠 pandemic 😷 distressed the world since the outbreak started. Most tragic is of course loss of human life in outrageous proportions. Words cannot describe the sense of grief that some experience
The loss that came to my mind this morning is the care that my partner Dr.David Hoffman has been delivering to kiddos of Neiva Colombia 🇨🇴 for the last nearly 3 decades, joined by my other partner and overall badass surgeon for nearly half a decade Dr. Lydia Lam.
The pictures above are the first baby that I operated on during my very first trip to Neiva. At this time of the year our office is usually hard at work putting the supplies together, fundraising and organizing the logistics of the mission of over 100 health professionals from , , , , to students and high school volunteers.
There are usually thousands of patients showing up for screenings from babies to young adults getting primary cleft care as well as secondary procedures including orthognathic surgery.
The virus stripped all those in need of the care they expected this year. Best we can do is pray and hope for the best and plan for a even stronger mission next year 💪🏼

The fallen heroes, the frontline HCW and residents in training: we hear you and stand with you.
Oral & Maxillofacial Surgeons are in one of the highest risks category for contracting coronavirus. This stems from the close proximity in which we work with patients, but also because working in the oral cavity can aerosolize the virus.
Due to the pandemic, Oral & Maxillofacial Surgeons are stopping elective surgery and committing to only providing emergency care.
OMFS Emergencies include:
-Infections
-Uncontrolled pain
-Bleeding
-Facial Trauma
-Head & Neck Cancer
To minimize the risk of spreading COVID19, surgeons as well as all other heathcare providers, need full PPE that includes a gown, N95 mask, full face shield, hair covering, gloves and booties. Any procedure that produces aerosolization (drilling) is performed in a negative pressure room.
We are doing our part to conserve precious PPE while keeping our patients comfortable and out of the emergency room so that the front line providers can focus on the sickest patients.
PPE continues to be in shortage across the US. Please do your part and donate any PPE that you have to
