Another success story , patient is going home !!!!!

Darlene came to Parkhouse Rehabilitation Center following a hospitalization due to Upper Respiratory Infection. Prior hospitalization Darlene was living with her daughter.

Upon evaluation following her hospitalization, Darlene requires mod/max A for transfers , ambulation and max assistance with activities of daily living and requires front wheel walker for mobility with her determination and support of her family Darlene was able to return to her prior level of function in all areas and is back to using RW for ambulation.

Darlene returned home to live at supervision level with her daughter and grandkids and she is looking forward to the holidays at home . Great job Darlene !!

Norma came to Parkhouse Nursing and Rehabilitation Center after a fall at home .
She needed to regain her strength so that she could resume her activities of daily living without assistance.
At the time of admission, Norma required assistance for all their functional mobility and self-care tasks. Norma required education to adapt and manage her new daily routines for her recovery.
Norma actively participated in skilled physical and occupational therapy to increase her functional independence and return home safely.
Her personalized rehab plan addressed and improved her balance, gait, and ability to navigate stairs safely. Additionally, Norma worked to improve her ability to bathe, dress, and groom independently.
Norma achieved her goals and was able to go home with her independence restored. She no longer needs a wheelchair , though she may require a walker when walking outdoors.

We appreciated the opportunity to participate in the Phoenixville Area Senior Center’s Annual Health Fair last evening. It was rewarding to engage with our local seniors and share information about our upcoming Holiday Bazaar on November 22 from 10 am to 3 pm. The Bazaar is open to everyone in the community, featuring local vendors and refreshments.
