In this role, the LVN will provide coordinated skilled nursing care to pediatric patients in their place of residence. Nurses will demonstrate accountability and responsibility in collaborating with the team (TPH staff, patient/family, and referring agency) to establish and achieve patient goals and maintain high quality patient care. These services will be performed in accordance with physician's orders and under the supervision of the Director of Nurses.

We are looking for individuals who are compassionate and comfortable working within patient's homes or traveling to and from school as well. Qualified candidates will possess:
A certificate/degree from an accredited school of vocational nursing.

Reliable transportation and proof of current and valid auto liability insurance.

The LVN nurse will monitor the patients' daily activities and perform medical duties such as taking the patient's temperature, administering medications, monitoring food intake, helping with bathing needs, and responding to the patients' inquiries, concerns, and requests etc. Please be familiar with PEG/PRN vent.
