Being involved in community outreach means more than simply offering services—it means building trust, listening closely, and meeting seniors where they are, both literally and figuratively. It reflects a commitment to bringing access to quality care into the places they call home, so that older adults can receive compassionate, timely, and dignified support without the added burden of navigating unnecessary barriers.

This mission becomes even more poignant when many of these seniors are beneficiaries of the state’s Medi-Cal programs, underscoring the importance of ensuring that vulnerable older adults have meaningful access to the high-quality care, support, and human dignity they deserve. Through outreach, education, and presence in the community, we help seniors and their families feel seen, supported, and connected to care that preserves independence, promotes well-being, and honors them in the places where they live.

Senior Care Clinic House Calls is proud to welcome Dr. Mirian Obioha, DNP, PHN, FNP-C, as the newest member of our growing team of clinicians delivering medical house calls to vulnerable, homebound seniors in the community. A graduate of the Betty Irene Moore School of Nursing at UC Davis Doctor of Nursing Practice (DNP) program, Mirian brings more than a decade of progressive nursing and advanced practice experience across geriatrics, primary care, and post-acute care, with a strong foundation in patient-centered, evidence-based practice.

Her leadership background includes service as a Director of Nursing (DON) in skilled nursing facilities, where she led annual state survey readiness, strengthened regulatory compliance, and improved quality outcomes—including elevating a facility from a 2-star to a 3-star rating and maintaining a 5-star CMS rating. She oversaw nursing operations encompassing staffing, recruitment, and compliance, and played a key role in implementing COVID-19 emergency response and isolation protocols in alignment with CDC guidance. Earlier in her career, she served as a Nurse Consultant, developing and implementing individualized care plans and providing caregiver education on infection control and medication administration. This depth of clinical leadership and operational expertise uniquely informs her approach to home-based primary care, supporting safe transitions, preventing avoidable hospitalizations, and advancing high-quality care for seniors aging at home.

We are honored to welcome Dr. McKenna Van Der Haegen, DNP, FNP-BC, CNL, as the newest member of our Senior Care Clinic House Calls family.

McKenna brings a rare combination of clinical excellence, global service, and deep compassion for older adults. Her training through the Betty Irene Moore School of Nursing at UC Davis, her oncology and infusion expertise, and clinical rotations—including geriatric medicine, home-based primary care, and international mobile medical camps—reflect a provider shaped not just by skill, but by heart, humility, and purpose.

Her doctoral scholarly work advancing sexual-health screening for oncology survivors demonstrates her commitment to elevating quality of life across the lifespan, especially for those whose voices are often overlooked.

McKenna joins our team with a spirit perfectly aligned with our calling—care that is relational, respectful, and delivered where it matters most: in the patient’s home. Her background in complex IV therapy, oncology navigation, global health outreach, and person-centered education equips her to serve our seniors with both clinical confidence and profound humanity.

With a career rooted in compassion and clinical excellence, Mallorie Hogan, MSN, FNP-BC, WCC, has built her professional life around advancing patient care across diverse healthcare settings. From her early service as a volunteer in the UC Davis Emergency Department to her years as a Charge RN at a skilled nursing facillity, Mallorie demonstrated a steady commitment to both patient safety and team leadership. Her ability to balance direct bedside care with supervisory responsibilities—overseeing LVNs and CNAs while ensuring timely interventions—helped shape her into a practitioner who values both the human and technical aspects of medicine.

Mallorie’s recent roles as Nurse Practitioner, Clinical Nurse Manager, and Assistant Medical Director at a home health agency further highlight her leadership in home health and community-based care. She has managed responsibilities ranging from vaccine coordination during the pandemic to supervising COVID-19 testing programs, reflecting both adaptability and public health commitment.
