At Asbury Park Nursing and Rehabilitation Center, we recently had the privilege of caring for Mr. N., who was admitted to our facility.
Upon admission, Mr. N. required moderate assistance with mobility and activities of daily living. Through an individualized rehabilitation program developed by our interdisciplinary team, including physical and occupational therapy, Mr. N. demonstrated consistent and meaningful progress. Over the course of his stay, he regained lower extremity strength, improved postural stability, and advanced to ambulating independently with a cane.
In addition to mobility gains, Mr. N. achieved increased functional independence with self-care tasks such as dressing, bathing, and meal preparation. With ongoing encouragement and support from our skilled therapy staff, he was successfully discharged home, equipped with the skills and confidence necessary to resume independent living safely.

Please join us in congratulating Mr. N. on his remarkable progress and wishing him continued health✨

At Asbury Park Nursing and Rehabilitation Center, we are honored to share the inspiring journey of Mrs. J, who was admitted following a thoracolumbar (T/L) washout, recurrent falls, and chest pain. Upon arrival, she required extensive assistance with mobility and self-care.

Through a personalized and compassionate rehabilitation program tailored by our physical and occupational therapy teams, Mrs. J made steady and meaningful progress. Over the course of several weeks, she regained strength, improved her balance, and advanced from using a front-wheeled walker to walking independently with a slow and steady gait—no assistive device required.

With ongoing encouragement and expert guidance, Mrs. J also regained independence in essential daily tasks such as dressing, bathing, and meal preparation. Her determination, combined with the support of our dedicated team, has prepared her to safely return to independent living.

We are incredibly proud of Mrs. J’s hard work and grateful to have been part of her recovery journey. These moments are a true reflection of why we do what we do. 💙

We are proud to announce that Asbury Park now offers dedicated Transport Van services to support our residents with timely and reliable transportation to their most important medical appointments. Whether it’s dialysis, cardiology, oncology, wound care, or other specialty visits, we are committed to ensuring that appointments are kept, not canceled, so residents receive the care they deserve without interruption.

This service is also helping empower our residents with greater discharge planning options, as we now provide transportation to Room & Boards and Board & Care facilities, giving them more flexibility and choice in their next steps.

This investment in transportation is just one more way we are raising the standard of care and improving outcomes.

Sommai Shelton, RN, MDS Director at Asbury Park
We are proud to shine a spotlight on our incredible MDS Director, Sommai Shelton, RN, whose passion for nursing and dedication to her team and residents make her a cornerstone of our community. Sommai was born and raised in Nakornsawan province, Thailand, where her calling to care for others began at a young age. She graduated in 1980 with a four-year nursing degree, specializing in ICU, Medical-Surgical, Pediatrics, Emergency Medicine, GYN/Labor, and Psychiatric Nursing. With a deep love for people and a natural gift for caregiving, Sommai has spent her life lifting up others through her work in healthcare. As the oldest of four siblings, Sommai’s nurturing spirit was evident early on—and that same spirit continues to guide her leadership today. She has been a dedicated member of the Asbury Park family for the past seven years, leading with strength, compassion, and an unwavering commitment to excellence. Sommai describes Asbury Park as a beautifully multicultural environment, where nurses and staff bring rich backgrounds that reflect and support the diversity of our residents. “It feels like home,” she says. “Our team is friendly, our departments work hand-in-hand, and when we face critical challenges, we come together, arms around one another, to find the best solution.” Outside of work, Sommai has been happily married for 29 years, is a proud mother of one son, and a joyful grandmother to her 8-year-old granddaughter. She loves cooking, gardening, and traveling, and brings the same warmth and care to those around her at work as she does with her family. We are so grateful to have Sommai as a part of our leadership team—her wisdom, experience, and heart make Asbury Park stronger every day❤️
