Gold and gold ceramic crowns on the back teeth are still the ultimate. My patients can get 20-30 years life span on this type of hand made crown. Zirconia and lithium desilicate won’t last that long. Plus they require more tooth reduction. Tooth structure is precious.

Christmas with our family and friends is a happy time. But happiness is with us throughout the year. Two patients of mine. One involved the entire lower teeth & one was a single upper front tooth. Which one was more of a challenge? It was the single tooth ceramic replacement. To match it exactly with the adjacent natural teeth is very difficult but after a few try ins & shade/characterization changes we did it. Now we can replace the nearby non-matching ceramic when my patient is ready! The lower case was to restore the worn lower front teeth from a lifetime of bruxism (grinding & clenching). We have to obtain space so I bonded composite (acrylic) to the tops of the back teeth then I could add composite to the fronts. This was done in one long appointment. We will wait months & adjust the bite as needed before replacing the composites with ceramic.

I don’t always show the extreme cases but here is one we just completed. There is no caries (cavities). Asymptomatic. The enamel over time has been breaking off. My diagnosis is enamel hypoplasia. The treatment was full coverage bonded ceramics.

After eight years today was the case completion for my patient from Montana. This only shows the upper anterior restoration but it was a full mouth rehabilitation including orthognathic surgery with my interdisciplinary team consisting of a maxillofacial surgeon (the only team member from Montana), periodontist (implant surgeon) and an orthodontist.

Gingiva (gum) recontoured & composite resin bonded to edge of tooth #8. A very nice result. Often patients will feel their teeth are short when actually the gums are too high (altered passive eruption).
