Coalition for Compassionate Care of California CCCC promotes high-quality, compassionate care for everyone who is seriously ill or nearing the end

Since 1998, CCCC has played a unique and vital role in improving care for people with advanced and terminal illness. We understand that changing the model for end-of-life care involves not only families, but healthcare providers and state policy makers. CCCC is tackling this issue on all three fronts, addressing the diverse needs of patients and loved ones, medical professionals and public policy makers. California has always been an innovator in end-of-life care, and CCCC is building the foundation to make real change happen in the many ways:

• We provide our expertise, tools and resources to families, patients, policy makers and healthcare providers to help ensure that all Californians receive high-quality palliative and end-of-life care.

• We lead efforts to train healthcare professionals about Physician Orders for Life-Sustaining Treatment (POLST), as well as cultural diversity and end-of-life care in California. We help them develop the skills they need to talk with seriously-ill patients and families facing decisions about goals of care. http://capolst.org/

• We work with and support local coalitions throughout the state that provide support and education to healthcare providers and those facing treatment and care decisions.

• We bring together people with a passion for increasing access to palliative care to spark growth of community-based palliative care throughout California.

Our free materials include multilingual POLST forms, FAQs, conversation scripts, model policies, and guidance to support high-quality, person-centered POLST discussions. Providers looking to deepen their knowledge can also join the POLST Champion Certificate Program, which offers comprehensive and accessible training on best practices for POLST completion and use. Participants may complete the full certificate or take individual courses as needed, with discounts available to CCCC members.

These tools include free palliative care explainer videos, disease-specific guides for cancer, dementia, heart disease, and lung disease, and caregiver supports focused on communication and decision-making. For more complex clinical decisions, CCCC’s certified decision aids offer clear, multilingual guidance on CPR, ventilation, tube feeding, and artificial hydration, helping families understand their options and document preferences.

These free tools include guidance on advance directives, healthcare agents, sample forms, FAQs, and practical conversation resources that make ACP easier to integrate into everyday practice. To further strengthen clinical skills, we also offer short, on-demand ACP courses developed with the CSU Shiley Haynes Institute for Palliative Care. Each course includes CE and focuses on effective ACP conversations, documentation, and billing.
