Coalition for Compassionate Care of California CCCC promotes high-quality, compassionate care for everyone who is seriously ill or nearing the end

Since 1998, CCCC has played a unique and vital role in improving care for people with advanced and terminal illness. We understand that changing the model for end-of-life care involves not only families, but healthcare providers and state policy makers. CCCC is tackling this issue on all three fronts, addressing the diverse needs of patients and loved ones, medical professionals and public policy makers. California has always been an innovator in end-of-life care, and CCCC is building the foundation to make real change happen in the many ways:

• We provide our expertise, tools and resources to families, patients, policy makers and healthcare providers to help ensure that all Californians receive high-quality palliative and end-of-life care.

• We lead efforts to train healthcare professionals about Physician Orders for Life-Sustaining Treatment (POLST), as well as cultural diversity and end-of-life care in California. We help them develop the skills they need to talk with seriously-ill patients and families facing decisions about goals of care. http://capolst.org/

• We work with and support local coalitions throughout the state that provide support and education to healthcare providers and those facing treatment and care decisions.

• We bring together people with a passion for increasing access to palliative care to spark growth of community-based palliative care throughout California.

Language should never be a barrier to compassionate care.

The Coalition for Compassionate Care of California (CCCC) has expanded its Patient-Centered Decision Aids to help clinicians engage patients and families from diverse backgrounds.

These trusted tools—supporting conversations about life-sustaining treatment options—are now available in English, Spanish, Chinese, Punjabi, Russian, Tagalog, and Vietnamese, with Pediatric Decision Aids for CPR, Ventilation, and Tube Feeding in English and Spanish.

Certified by PALTmed, CCCC’s Adult Decision Aids meet rigorous clinical standards for hospitals, hospices, and skilled nursing facilities. Digital and print access options are available, with enterprise-level subscriptions for organizations and systems.

The 17th Annual Coalition for Compassionate Care of California (CCCC) Palliative Care Summit was a tremendous success thanks to our engaged attendees, inspiring speakers, and generous sponsors.
