For individuals with memory impairments, we provide resident-centered care and lifestyle support in our self-enclosed neighborhood where flexible daily routines are promoted. We participate wi

th Medicare, Medicaid, Managed Care and Private Pay as sources of payment options. As a dedicated provider of care to our veterans, we are a contracted provider of the Veterans Administration. We are conveniently located, with easy access from Route 6/19 and Highway 86.

Success Story Spotlight ✨

When Ed arrived at Crawford Care Center, he was facing multiple challenges. From weakness and balance issues to a recent diagnosis of pneumonia.

But Ed had a goal: to return home to his wife. ❤️

Through hard work and dedication, he made steady progress with therapy, improving his balance, strengthening his mobility, and regaining independence with daily tasks. Step by step, he pushed forward.

By discharge, Ed was able to walk up to 100 feet with a walker and even navigate stairs- huge milestones on his journey home.

And the best part? He made it home. 🏡

We’re so proud of Ed’s determination and the teamwork that helped make it possible.

Nothing beats seeing smiles, hearing laughter, and creating sweet memories together 💛

Success Story Spotlight ✨

Betty came to Crawford Care Center after a fall at home that resulted in a hip fracture- facing not only physical challenges, but also a fear of falling again.

With determination (and a little grit), Betty got to work. 💪
Our therapy team supported her in rebuilding strength, regaining confidence with walking, and mastering stair training, helping her take back her independence step by step.

Even when faced with a setback during her stay, Betty refused to slow down. Her resilience never wavered, and her focus stayed clear: getting back home.

And she did just that. 🏡✨

We’re so proud of Betty and the incredible progress she made. Stories like hers are a powerful reminder that strength isn’t just physical...it’s a mindset.
