Job Summary
The Home Health Aide plays a crucial role in providing compassionate and personalized care to individuals in their homes. This position involves assisting clients with daily living activities, ensuring their comfort and safety, and promoting their overall well-being. The ideal candidate will have experience in patient care, as well as the ability to work effectively with individuals with various needs, including those with developmental disabilities and dementia.
Duties
Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
Monitor and document vital signs, report any changes in patient conditions to healthcare professionals.
Provide companionship and emotional support to clients, fostering a positive environment.
Administer medications as prescribed and assist with meal preparation and dietary needs.
Maintain cleanliness in the client’s living space, including laundry, cleaning, and cooking as necessary.
Utilize EMR/EHR systems for accurate medical documentation of patient care activities.
Assist clients with mobility using equipment such as Hoyer lifts when necessary.
Provide specialized care for patients with Alzheimer’s disease or other forms of dementia.
Support hospice care efforts by providing compassionate end-of-life care when needed.
Ensure compliance with HIPAA regulations to maintain patient confidentiality.
Skills
Strong patient observation skills to identify changes in health status.
Experience in assisted living or nursing home environments is preferred.
Knowledge of medical terminology and familiarity with CPR techniques.
Ability to perform catheter care and understand the needs of patients requiring long-term care.
Experience working with people with developmental disabilities or those requiring memory care.
Proficient in caregiving practices within home health settings, including cleaning and meal preparation tasks.
Excellent communication skills to effectively interact with patients, families, and healthcare team members.
