PLEASE ALWAYS BE AWARE OF THE QUALIFICATIONS OF THE PERSON TREATING YOU AT WHATEVER HOSPITAL, CLINIC, EMERGENCY ROOM, URGENT CARE OR WALK-IN CLINIC YOU END UP AT. AND MOST IMPORTANTLY MAKE SURE YOU MEET THE MD OR DO IN CHARGE OF YOUR CARE!

In this day and age there is a lack of transparency in medicine. It has become a business and for that reason business people are starting to replace Medical Doctors for a cheaper alternative even if the qualifications are not comparable. Not only that, some of these nurse practitioners or physician assistants (so call mid-level provider) attend “online programs where open book test are allowed” and never have any in hospital clinical rotations. Yes, nurse practitioners are vital but they are not Medical Doctors and always, always, they need to be supervised by an MD or DO on the premises. According to the American Medical Association,
“Nurses are indispensable but they cannot take the place of a fully trained physician”

Don’t let corporate greed put you or your family at risk. Please don’t be fooled by the business people running the show. Please inform yourself. I am not making this up! Don’t be a victim!

When a patient want our services, we from the very beginning strive to provide for the patient the care that every patient looks for and deserve. They come with questions, sometimes fearful and anxious about their condition. I can't say enough about Michael Lugo and his Steak House and Pesca Rooftop Kitchen and Bar inside The Hyatt Hotel in Vilano Beach at 117 Vilano Road in St. Augustine Florida. What an excellent service and quality of food. We celebrated there this evening our oldest son in law Lex birthday with friend and family. This was planned by Josephine our oldest daughter, for her husband and she invited friend, coworkers and family. We highly recommended it. We wish Michael Lugo all the success!

INTERESTING PATIENT I JUST TREATED AT MY OFFICE.

A very nice patient referred to my office by the primary care physician because the patient continue to complain of left hip pain even after multiple visits to a local orthopedic group's office. Every time the patient went there all he would see was a Physician Assistant (PA) and never a doctor. The PA would always take a hip xRay and tell the patient you have no arthritis in your hip, what you have is trochanteric bursitis and would give the patient a cortisone injection. Nothing else. This routine happened several times.

When the patient came to our office after a complete history and physical to address her condition I immediately suspected a tear.

We then proceeded to order an MRI of the left hip and immediately we knew what was causing her problem. She had an almost complete tear of the gluteus minimus tendon, and a partial tear of the gluteus medius and maximus and a small labral tear. These muscle tears are very common in the elderly, specially in females. The repeated cortisone injections the patient was getting were just making the condition worse. Why did they not order an MRI? Good question.

WHAT GOES ON AT MANY MEDICAL OFFICES AND WHY

You may have already experienced the sad reality of medicine now a days in many medical offices. Is what I call the “rat race of medicine”! I will exoplain to you what I am talking about and what has brought it about also.

Here is an example. You call the doctor’s office for an appointment and they give you an appointment for two or three weeks down the road or maybe more. Finally, the appointment day arrives and when you get there at your appointment time, you still have to have to seat and wait at time thirty or forty minutes or more before they come and take you in the exam room. After the nurse weights you and takes your vital signs, you wait some more. Finally, someone walks in the door, either a “Nurse Practitioner or a Physician assistant” (the so-called mid-levels), not a medical doctor and he or she does a rushed history and physical exam, orders some tests and asks you to come back in a week to ten days for a follow up. You never have seen the doctor yet. You then go home kind of disappointed that you never got to see the doctor you actually came to see. You then come back for the follow up as instructed, and again the “mid-level” starts you in a trial treatment of something. If you are lucky the doctor peaks in the door for a minute or two and then he is gone without giving you a chance to ask any questions you may have. Now again before you leave, they schedule you for another follow up in a couple of months to do more test and see how the treatment instituted is going. And like this, they keep you going in and out thru the “revolving doors” over and over doing more and more of the most well-paid test or procedure by the insurance company. This is not personalized medical care like the old days! This is revolving door medical care! They got to make their money somehow to stay in business and not have to close their doors!

Why has that scenario become so popular you may ask? Well, here is the reason. Since about 1985 to now (40 years) just Medicare payments to physicians have been cut by about 40 to 45% and the rest of the health insurance has followed suit. All this cuts in payments and at the same time the cost of running a practice which has more than triple, have had a great impact on medical practices. So, what have the physicians been forced to do to stay in practice? They have to see more and more patients, use less and less qualified assistants that cost less and do more and more test of the ones that pay well. Who suffers in this rat race? The patients!

To avoid burn out and to avoid having to close their practice, this has led many physicians to abandon the “insurance medicine rat race” completely since they are paying less and less every year, while physicians are needing a huge office staff to run a high-volume revolving door type practice. In addition, they have noticed that treatment and test that are really helpful and work and are even less damaging to the body, are not covered by any insurance companies. Examples include, Stem Wave treatment, Prolotheray, Platelet Rich Plasma (PRP), Micro fragmented fat (MFAT) and Bone Marrow Concentrate (BMC) injection procedures. We primarily do minor surgical interventional procedures such Prolotherapy, Platelet Rich Plasma, Bone Marrow MSC and other joints and spine injection. With that been said, if what you are looking for are narcotics or other controlled substances, don’t expect to get them in this office. Don't waste my time or yours. Or your money for that matter!
