NOTICE ST JOSEPH SPECIALTY CLINIC PATIENTS
Effective June 1, 2022 we will no longer accept new patients. Joseph practice will be closing permanently on June 30,
2022. Dr Havaldar and Dr Shewell will continue to see patients in:
Bethany, MO [Harrison County Community Hospital] and
Chillicothe, MO [Hedrick Medical Center/St. Endoscopy and Surgical reports and pathology are on the Mosaic
Electronic Records that you can access via your portal or Medical
Records, without charge. Reports of consultations requested by P*Ps were sent to your P*P. Medical Records are available from us,
after signing a HIPAA release, at a cost of $24.85 and 57 cents per
page payable prior to obtaining records using a Cashier’s check. After July 1, 2022 medical records can be requested by email at
specialtyclinic@icloud.com or by mail to PO Box 8657,
