Our Story

Avamere Transitional Care at Sunnyside resides in a quiet residential neighborhood with a big back yard for all to enjoy. Our caring team of skilled nurses and rehabilitation therapists work closely with our Salem rehab center residents, family members, and medical staff to provide a personalized senior care program for Salem seniors who are recovering from illness or injury. Not only do they help restore our residents’ health, but their spirits too. All of these activities are meant to brighten a senior’s day, something that Avamere Transitional Care at Sunnyside does especially well. Also note, Avamere Transitional Care at Sunnyside is both licensed and Medicare/ Medicaid certified.
