What I Tell Patients About Smartwatches and Health Data

A lot of patients ask me:

“Can I trust my smartwatch?”

Short answer: Sometimes. But it depends on what you’re looking at.

Here’s what the research shows:

✔️ Home blood pressure cuffs (the upper arm kind) are well validated. They actually improve blood pressure control when used properly.

⚠️ Smartwatch blood pressure estimates are not the same thing. They’re improving, but they aren’t yet a replacement for a cuff.

✔️ Smartwatch ECG features can detect atrial fibrillation reasonably well.
❌ They cannot diagnose heart attacks or replace a standard ECG.

✔️ Sleep trackers are good at telling you when you were asleep vs. awake.
❌ They are much less accurate at determining sleep stages.

✔️ Heart rate at rest is generally reliable.
⚠️ Accuracy drops during intense exercise or heavy movement.

The big picture?

Wearables are helpful tools. They’re not diagnostic devices.

They’re best used for:
• Spotting trends
• Encouraging healthy behavior
• Prompting conversations with your clinician

They shouldn’t replace proper testing when something serious is suspected.

Technology in medicine is expanding quickly. My job is to help patients use it wisely — not blindly.

If you use a wearable, bring the data to your appointment. We can look at it together and decide what matters.

Personal Glucose Monitoring Data and Patient-Generated Health Data: A Literature-Based Assessment of Clinical Utility, Accuracy, and Role in Medical Decision-Making

The integration of patient-generated health data (PGHD) into routine clinical care reflects a broader shift toward continuous, individualized health monitoring. Among forms of PGHD, personal glucose monitoring—whether via traditional fingerstick logs or continuous glucose monitoring (CGM) systems—offers one of the most mature and evidence-rich paradigms for assessment. A growing literature base evaluates its clinical utility, data accuracy, and impact on medical decision-making. Summarizing these findings provides insight into how PGHD can be responsibly and effectively incorporated into therapeutic strategies.

Clinical Utility of Personal Glucose PGHD

Numerous studies demonstrate that glucose PGHD can improve glycemic outcomes in both type 1 and type 2 diabetes when coupled with structured clinical interpretation. Consistent patterns of elevated or fluctuating glucose levels, captured outside of clinic visits, facilitate more personalized adjustments to medications, diet, and activity. CGM use has been associated with improved time in range (TIR), reduced hemoglobin A1c, and fewer hypoglycemic events compared with standard self-monitoring blood glucose (SMBG) alone, particularly in insulin-treated patients. Importantly, these benefits derive not only from data collection but from structured integration with therapeutic actions and patient education.

Beyond glycemic targets, PGHD can support detection of unrecognized nocturnal hypoglycemia, post-prandial excursions, and behavioral effects on glucose trends. Dynamic data empowers both patients and providers to refine lifestyle and pharmacologic strategies in real time rather than relying solely on periodic laboratory snapshots.

Accuracy of PGHD Metrics

Device accuracy is foundational to clinical utility. CGM systems have undergone rigorous evaluation against reference standards, with many meeting accepted accuracy thresholds (mean absolute relative difference [MARD] generally
