▪︎Policy Form → To make sure we’re on the same page about appointment scheduling, cancellations, and payments.
▪︎Health History Form → To tailor your massage safely and effectively to your body’s needs.

We know paperwork isn’t the most exciting part of your visit—but it’s important!
