Concho Valley Home Health Care is accepting applications for an RN / Case Manager position for both field / office. Must have at least 2 years of experience in the area of Home Health and case manager experience is a plus. This individual will be responsible for Care Coordination with physicians, patient's, family members, field staff as well as making field visit when necessary. He /she will work closely with our Clinical Director, Administrator, scheduling department and field staff to ensure quality care and continuity of care is delivered to our patients and clients. Will works directly under the Clinical Director and also, serve as Alternate Supervising nurse when Director is not in the office. Works indirectly under the Agency Administration.
