Our staff is professionally trained and operates holistically in developing an appropriate Plan of Care (POC), no matter your need. The POC and your feedback will be present for all to clearly understand and align care expectations. Beyond Faith Homecare operates directly under your doctors’ orders. The first thing we do when we receive a physician’ order is to conduct a Nursing Assessment to develop the (POC). We collaborate with your doctor, family and the rest of your healthcare team directly to provide clear communication, a schedule that works for all, quality and coordinated care.
Our expertise is in successfully and compassionately managing clients in need of complex medical care such as Cardiac/CHF, COPD/Asthma, Wound Care, Diabetic Management, Medication Management, Medical/Surgical Nursing, Parental and Enteral Nutrition Therapy, Physical, Occupational and Speech Therapy to companion care; assistance with Activities of Daily Living (ADL’s) and education.
