Happy Labor Day Y'all! I hope everybody if off to a great school year and has a fantastic long weekend.

I am making lots of progress on getting my website and subsequently my new practice started. I expect the website to be live by the end of this week or early next week and ready to provide care the first days of October!

In writing content for the website, I have had an opportunity to reflect on my career and the wonderful time I have had taking care of the best patients on the planet. I would be lying if I did not admit that I got a bit teary-eyed at moments.

I want to share with you a portion of my reflections that I think expresses my feelings well:

"In total now, I have spent over 30 years focused on caring for children and adolescents. I have had extensive experience in a variety of settings to include in the hospital, newborn nursery, delivery room, intensive care unit, and in the outpatient office. Each patient and family that I have taken care of has taught me something new about caring for people. Through these relationships, I have gained invaluable knowledge, wisdom, and perspective that you can not find in any book, website, or lecture. All of this has culminated in becoming what I believe is the best pediatrician I have ever been."

Thanks for spending your valuable time on this life journey with me over the years...

The time is finally here!

I finished with my employment job this past week! Now I am ready to commit full time to new endeavors.

I will be working over the next couple of weeks to get the new website up with information about all the new things that will be available. Please reach out through the website or here if you have any questions.

I hope y’all are having a great weekend! With all of the rain, I am seeing more children with mosquito bites than I have seen in a long time. In particular, many more kids are having quite a bit of redness and swelling. I am wondering if there is a particular variety of mosquito out there that we are reacting more to. I want to pass along some information for treatment and prevention of mosquito bites.

For treatment you should give benadryl liquid at the dose recommended on the label for 2 days after the bite. This will help reduce the amount of redness, swelling, and itching. Also put on some over the counter hydrocortisone cream 3 times per day until the bump is gone. This will help the bump from the bite resolve quicker.

For prevention the best two options are:

1). DEET 25%—this would be the “deep woods” version. Family and kid versions are 15% at most and may last only an hour or two. The deep woods strength lasts for 6-8 hours. It is safe and has been used for almost 70 years with no known harmful effects to humans. Just to be safe though, it is best not to apply to children’s faces or hands and wash hands and face before eating.

2). Picaridin- this is my favorite. It has been available since 2005 but a lot of people still do not know about it. The active ingredient is derived from piperidine found in black pepper. It comes in a spray or as a lotion. It has a pleasant odor compared to DEET and is not greasy or oily. An added bonus is that it repels insects for up to 12 hours!

My experience has been that once I share this information with patients most patients get far fewer insect bites. Hopefully this will help you all have a happy rest of the summer.

Hello again everybody! I hope you’re having a great week.

I have another poll question I would like to pose.

I am looking into the possibility of being available mostly through video calls, email, text, and phone calls to provide medical care to your children. This would be a consult service and would not replace your primary care pediatrician. I could provide second opinions, review medical records, manage ADHD, manage other behavioral and developmental concerns, counsel, and provide for other medical care that would be appropriate by teleconferencing.

To be able to do this, I would have expenses such as insurance and would not be able to accept health insurance plans. There would be different membership options, but all would be paid directly. I cannot give specific prices now, but I would keep the fees as low as reasonably possible. 

This service will be in addition to the plans I spoke of before.

Please let me know your thoughts, what your level of interest and any suggestions you have.

Hello Premier Pediatrics family! It has been a very long time since we have interacted. I want you to know that you all have been greatly missed. I really enjoyed knowing you and taking care of all the children of such good people.

I have been practicing pediatrics in San Antonio for the last year. Unfortunately I have not experienced the same level of enjoyment, satisfaction, and accomplishment as I did with Premier Pediatrics.

Thus, I have decided to leave my job to focus on doing things that improve the health of children and the lives of families on a larger scale.

My plan is to devote substantial time to interacting with parents and children through a media platform called Substack. On this platform I can write, post videos, have live interactive events, and produce podcasts among other things that you may find helpful. I anticipate this being much more impactful than what I did previously on Facebook as the Substack platform has much more flexibility and capability and I will invest the time needed to make this both educational and enjoyable.

Please let me know what you think about this and if you are interested in participating. I look forward to hearing your comments.

So in case you have not realized this already, there has been a tremendous amount of flu cases these last few weeks. I am working in urgent care and flu is by far the biggest problem out there.

The interesting part is that there are actually at least three different flu strains circulating. Two of the strains are Flu A and one is a Flu B. I have seen many, many people who are getting the flu and getting better and then getting another strain of the flu just two or three weeks after the first. Many times the second case is worse than the first because your body has not completely recovered before being reinfected.

The flu vaccine protects you against four different flu strains each year. All three of the flu strains circulating are in this year‘s flu vaccine.

The important message I want to leave you with that it is still beneficial to get a flu vaccine even if you have had the flu. In fact, even if you have had the flu twice, the vaccine can prevent you from getting flu a third time this year. Flu season lasts at least through April and into May.

Of course you should not get the flu vaccine while you are sick with the flu. Most people are able to get a flu vaccine a week after they are completely recovered from the flu infection. If you have any questions about timing based on your specific health conditions, you should ask your primary care doctor.

Well folks, the office is officially closed. I have felt a tremendous sense of loss in saying good bye to so many wonderful, long term patients and the office staff. I have been deeply touched by all of the kind words and appreciation you all have expressed. It is very rewarding to know that I have been able to make a difference in your children's and family's lives.

I look forward to continuing our relationships through interacting on Facebook other online resources to come later. Also, I invite you to direct message me on Facebook if there is anything I can help you with going forward. My next plan is to start our conversation on food additives and dyes. There is quite a bit of information evolving in this area so give me some time to do research so I can get back to you with good information.

As I have talked to many of you about, I will be working in Quick Care. This will be starting about mid December then going forward. I will be posting information about dates and locations and guidelines for what I can help you with closer to time so be on the lookout for that.
