Happy Monday everybody!

This is the big week for Premier Pediatric Consultants...the start of a new phase and a different (better) kind of healthcare.

Wednesday, October 1 will be the first day the new office will be open and available to provide care.

For those living in or commuting to the San Antonio area I am available for both in person and telehealth care. You can choose to have me manage a specific problem like Asthma, ADHD, depression, school difficulties, constipation, and other long term problems with problem focused appointments OR you can receive ALL of your primary care needs with a membership.

For those persons throughout Texas and Louisiana who are too far away to come for in person care, there are specific membership plans for telehealth care for any problem that can be reasonably done virtually.

Appointments are much longer, more thorough, and far more personal than you get in a standard practice. For membership patients I am always available by text, phone, email, and video for medical needs, concerns, and urgencies.

THE TIME HAS FINALLY COME! The website is completed and posted with full function! See the link below.

I am offering what is called Direct Primary Care. It is a type of practice focused on the relationship between patients and the doctor without the limitations imposed by big business and insurance companies. The website gives more information about this type of practice and more details about getting care.

For this first phase I am focused on providing care directly to individual patients. The next phase will come in the months ahead. During this time I will be developing educational and interactive multimedia to help keep our children healthy and happy.

I hope y’all are having a great weekend! With all of the rain, I am seeing more children with mosquito bites than I have seen in a long time. In particular, many more kids are having quite a bit of redness and swelling. I am wondering if there is a particular variety of mosquito out there that we are reacting more to. I want to pass along some information for treatment and prevention of mosquito bites.

For treatment you should give benadryl liquid at the dose recommended on the label for 2 days after the bite. This will help reduce the amount of redness, swelling, and itching. Also put on some over the counter hydrocortisone cream 3 times per day until the bump is gone. This will help the bump from the bite resolve quicker.

For prevention the best two options are:

1). DEET 25%—this would be the “deep woods” version. Family and kid versions are 15% at most and may last only an hour or two. The deep woods strength lasts for 6-8 hours. It is safe and has been used for almost 70 years with no known harmful effects to humans. Just to be safe though, it is best not to apply to children’s faces or hands and wash hands and face before eating.

2). Picaridin- this is my favorite. It has been available since 2005 but a lot of people still do not know about it. The active ingredient is derived from piperidine found in black pepper. It comes in a spray or as a lotion. It has a pleasant odor compared to DEET and is not greasy or oily. An added bonus is that it repels insects for up to 12 hours!

My experience has been that once I share this information with patients most patients get far fewer insect bites. Hopefully this will help you all have a happy rest of the summer.

Hello Premier Pediatrics family! It has been a very long time since we have interacted. I want you to know that you all have been greatly missed. I really enjoyed knowing you and taking care of all the children of such good people.

I have been practicing pediatrics in San Antonio for the last year. Unfortunately I have not experienced the same level of enjoyment, satisfaction, and accomplishment as I did with Premier Pediatrics.

Thus, I have decided to leave my job to focus on doing things that improve the health of children and the lives of families on a larger scale.

My plan is to devote substantial time to interacting with parents and children through a media platform called Substack. On this platform I can write, post videos, have live interactive events, and produce podcasts among other things that you may find helpful. I anticipate this being much more impactful than what I did previously on Facebook as the Substack platform has much more flexibility and capability and I will invest the time needed to make this both educational and enjoyable.

So in case you have not realized this already, there has been a tremendous amount of flu cases these last few weeks. I am working in urgent care and flu is by far the biggest problem out there.

The interesting part is that there are actually at least three different flu strains circulating. Two of the strains are Flu A and one is a Flu B. I have seen many, many people who are getting the flu and getting better and then getting another strain of the flu just two or three weeks after the first. Many times the second case is worse than the first because your body has not completely recovered before being reinfected.

The flu vaccine protects you against four different flu strains each year. All three of the flu strains circulating are in this year‘s flu vaccine.

The important message I want to leave you with that it is still beneficial to get a flu vaccine even if you have had the flu. In fact, even if you have had the flu twice, the vaccine can prevent you from getting flu a third time this year. Flu season lasts at least through April and into May.

Of course you should not get the flu vaccine while you are sick with the flu. Most people are able to get a flu vaccine a week after they are completely recovered from the flu infection. If you have any questions about timing based on your specific health conditions, you should ask your primary care doctor.
