Home Health is for patients who are recovering or managing a chronic illness. The goal is to improve health and regain independence with services like:

Wondering who qualifies for hospice care, and how to pay for it? You’re not alone.

At Heart to Heart Hospice, we’re here to make the process clear and compassionate. Hospice care is available to patients with a life-limiting illness, typically with a prognosis of six months or less, as certified by a physician.

The good news? Most hospice services are fully covered by Medicare, Medicaid, and many private insurance plans, including medications, equipment, and support services.

Our San Antonio CER team is out this week connecting with hospital Case Managers to share what makes the Heart to Heart Difference! While a hospice conversation can be initiated by anyone, hospice is often first discussed in a hospital setting. We’re committed to making those conversations more comfortable and helping clear up common myths about hospice care.
