Yesterday was all about enteral nutrition for our interns.
We were joined by Angela, one of our clinical nurse educators and Rachel, one of our nutrition support dietitians, who guided us through nutrition support from start to finish: tube type/selection/securement, placing tubes, selecting formulas, calculating TF prescriptions (simple-complex [hello propofol!]), and managing complications.
With the exception of placement, our interns will be come proficient in these skills by graduation.
That’s a wrap on orientation week 2022! We’ve explored our campuses and packed a lot in, including: supplement introduction and tasting, a tour of the amazing (one of our primary vendors), introduction to patient interviews, and rounded out the week with our hands-on NFPE workshop. Now off to start rotations next week 🎉

Yesterday our interns had a tube feeding skills lab with our friends at Coram Infusion Services. This ‘lab’ allows our interns to get hands-on experience with pumps, gravity bags, bolus syringes, enteral tubes, and several connector devices in a safe setting. We also got exposure to the role and knowledge of DME dietitians!
Preceptors are the cornerstone of any dietetic internship. We could not be more appreciative of all of the wonderful preceptors we’re lucky enough to work with. Thank you preceptors!

2021 Orientation Week ✅ We concluded our jam-packed orientation week schedule yesterday. Thursday and Friday were spent at the SIM center on our medical school campus to practice patient interviews and the nutrition-focused physical exam before rotations start next week. We’re looking forward to another great year!

Interns Morgan and Tiffany scrubbed up yesterday in the IR (interventional radiology) department to observe a PEG placement. Our interns see a large number of patients with PEGs or G-tubes throughout their clinical rotations, so being able to observe this procedure drives home the impact we have on the nutrition care of our patients.
Under these masks are some smiling faces. As patient-facing clinicians, our interns were given the opportunity to get their first dose of the COVID vaccine 🎉
The case study presentation is the culminating project of the clinical rotation. Interns Kim and Molly presented cases on Mucositis in patients undergoing Stem Cell Transplant (SCT), and Nutrition interventions in Cardiac Transplant complications.

Interns Mandy and Tiffany transformed our cafe yesterday with Asian inspired flavors for their theme meal project, and everything was a hit! Mandy wore her traditional Vietnamese dress for the occasion.
NFPE workshop, but make it virtual! We held our 5th annual workshop in person for our own dietetic interns and the San Diego VA interns, but were also able to successfully bring a virtual version in real time to 50+ participants nationwide.
A big thanks to our clinical nutrition staff and to everyone who was able to join!

Liver and Kidney Transplatation

During my fourth week of clinical, I had the opportunity to work in the UC San Diego Health outpatient kidney and liver transplant clinic. The clinic has a specialized healthcare team for both kidney and liver patients. I really enjoyed being a part of such amazing and collaborative teams for the week, and seeing all of the good that can come out of transplantation. Patients who are seeking transplantation come to the clinic and are evaluated by a dietitian, nephrologist or hepatologist, surgeon, coordinators, financial coordinators, pharmacists, and a social worker. After the initial visits, the team collaborates to determine the candidates to be added to the transplant list. Once added, both of the kidney and liver dietitians help to prepare patients for transplant by providing diet recommendations for either weight loss or gain, and by monitoring nutrition-related labs.
The liver transplant team is able to provide a deceased donor transplant, split transplant (one donated liver split for two recipients), or domino transplant (one donated liver transplanted to a recipient, and then the liver from the recipient transplanted to another recipient).
The kidney transplant team participates in The National Kidney Registry's Paired Exchange Program, allowing donors, who do not match their loved one, to exchange with a compatible donor. The team is also able to offer Hepatitis C transplants with minimal risk treatment after transplantation, steroid avoidance, transplant for patients with a BMI over 40, patients over the age of 70, and kidney autotransplantation. The UC San Diego Health kidney transplantation program has performed over 3,200 kidney transplants since 1968!
Nutrition in the ICU

As I progress through clinical rotations, I have been able to spend two weeks in the Medical and Surgical Intensive Care Unit (ICU) at UC San Diego Medical Center, Hillcrest. In a hospital, the ICU is home to some of the sickest patients often facing complications from surgery, accidents, infections, and/or severe breathing problems. It is a place packed with monitors, IV lines, feeding tubes, catheters, breathing machines, and a place where everything can change within seconds.
My favorite part of this rotation has undoubtedly been the morning “huddles”. Every morning, the entire ICU team consisting of the attending, residents, fellows, dietitian, speech therapist, physical therapist, respiratory therapist, and nurses gather and discuss each patient. This allows for a multidisciplinary, whole systems approach where health professionals from different disciplines can gain new perspectives and work together to provide the best possible care.
During my time in the ICU, I have learned about different tube feeding formulas and their indications, seen how different medications can affect the GI tract, learned about ventilators and indirect calorimetry, and practiced the art of piecing together a patient’s nutrition history from family members and medical charts to develop a thorough plan. Most importantly, I’ve been able to witness how proper and timely nutrition can positively impact these patients that are facing some of the most critical and challenging moments in their lives.
