Partners collaborates with physician networks, health plans, community-based organizations, and federal and state agencies to deliver programs and services that protect and support adults with complex health and social services needs, frail elders, people with disabilities, caregivers and families. Our President & CEO, June Simmons, along with Community Care Hub Managers Zoe Waters and Marlene Smith, met with peers across the Aging Network, share ideas, and learn from others working to strengthen services for older adults.

July 14, 2025 marks the 60th anniversary of the Older Americans Act (OAA)—historic legislation that laid the foundation for how our nation supports older adults in aging with dignity, independence, and connection.

As an agency, we are proud to serve older adults and individuals with complex health needs. Through community wellness programs, meal services, transportation, caregiver support, and more, we help high need populations stay healthy, engaged, and supported in the communities they call home.

We’re honored to stand alongside so many others committed to advancing the well-being of older adults.

Today marks the 60th anniversary of the Older Americans Act, signed into law on July 14, 1965. In California, this law continues to support programs that help older adults stay connected and independent — including meals, transportation, caregiver support, and more.

Services are available to older adults, people with disabilities, and caregivers regardless of income.

As a Social Work Care Manager, Maria goes above and beyond to support MSSP participants—not only by offering guidance but also providing unwavering emotional support during some of life’s most difficult moments.
