Partners in Care Foundation We are innovators in the social and behavioral determinants of health. Partners is proud to be an LGBTQ friendly organization.

Partners collaborates with physician networks, health plans, community-based organizations, and federal and state agencies to deliver programs and services that protect and support adults with complex health and social services needs, frail elders, people with disabilities, caregivers and families. Our programs have been demonstrated to significantly reduce costly hospital re-admissions, ED visits, and nursing home placements, resulting in improved health outcomes at lower cost through better coordinated care.

Congratulations to Lorena "Lore" Lopez on being named Employee of the Month for October 2025.

Lorena Lopez has been a vital part of Partners In Care Foundation since joining as a Housing Navigator. With over a decade of experience in Human Services, she brings a wealth of knowledge and compassion to her role. Lore’s work focuses on helping unhoused individuals secure permanent housing and creating personalized Housing Stability Plans, always collaborating closely with Enhanced Care Management teams to meet each person’s unique needs.

Before joining Partners, Lore made a significant impact at Grandma’s House of Hope in Santa Ana, where she led housing workshops, maintained strong landlord relationships, and supported clients transitioning from emergency shelters toward long-term housing success. Lore’s approach goes beyond casework—she’s a trusted team player who brings cultural awareness, technical skills, and a collaborative spirit to everything she does. She’s known for her empathy, clear communication, and reliability, especially in challenging situations. Her passion for empowering underserved communities truly embodies the values of Partners In Care Foundation.

ECM participant, Artur, was facing deportation—with nowhere to turn.

His work authorization had lapsed, and there was no public help available. That’s where we stepped in. Artur's care coordinator, Jesse Hernandez, helped him apply for Partners' Urgent Needs Fund.

Thanks to donor support, Partners Urgent Needs Fund made it possible to move quickly—providing flexible, immediate aid when no other resource existed. It’s how we say yes when everything else says no.

Congratulations to Danny Gomez for being named Employee of the Month for October 2025!

Danny has been a standout at Partners since he joined in 2018. He began as a Data Analyst and has steadily worked his way up to become the Manager of Analytics. It's been great to see his growth—not just in his technical skills, but also in how he's developed as a leader. Over the years, Danny has shown what strong leadership looks like. He's taken on challenges and turned them into opportunities for the team to improve. He's mentored several team members, helping them grow in their roles, and under his guidance, the team has made big strides—especially in automating many of their tasks. Thanks to his efforts, the team has been able to handle more work without needing to add extra staff.

What really sets Danny apart is the way he goes beyond just doing the work. He's focused on building a team culture that encourages growth and learning. He knows how to spot talent and help people reach their potential. That approach has made the whole analytics team stronger. People often say how approachable he is, how clearly he communicates, and how reliable he is when things get tough. We are pleased to announce the 2026 recipient of Partners' Lifetime Achievement Award: Leonard D. Schaeffer, Founding Chairman & CEO, WellPoint (now Elevance Health)

Mr. Schaeffer was Chairman & CEO of WellPoint from 1992 through 2004 and continued to serve as Chairman through 2005. Under his leadership, WellPoint was selected by FORTUNE magazine as America’s “Most Admired Health Care Company” for six consecutive years.
