Are you offering a D-SNP plan?



Dually-eligible older adults have 2X the risk of dementia, making early detection a critical component of care. In California, D-SNP policy highlights the Dementia Care Aware program as an approach to screening and as an overall resource for dementia care.



We have the training and the tools to help all D-SNP members get timely screening and their providers to get support in all aspects of dementia care.
