Dually-eligible older adults have 2X the risk of dementia, making early detection a critical component of care. In California, D-SNP policy highlights the Dementia Care Aware program as an approach to screening and as an overall resource for dementia care.



We have the training and the tools to help all D-SNP members get timely screening and their providers to get support in all aspects of dementia care.

Early detection supports whole-person care if we can connect people with dementia to the right support.

That’s why we want to highlight CalAIM. CalAIM’s ECM and CS programs help older adults living with dementia get elements of high-quality care for this population.
