On Friday, August 18, the ACTCM Clinic concluded almost 43 years of offering holistic, often primary health care to the community. At our height, we administered over 20,000 treatments a year, here at our main clinic, our many offsites including the Women's Resource Center, CPMC, Glide Memorial; and our various pop-ups including the San Quentin Health Fairs, the Skyline College Student Health Fair, and more. In the attached picture is a Gratitude Wall compiling notes and cards from patients expressing what their care has meant to them throughout the years.
As we are now concluding operations, everyone here would like to thank you for choosing our humble clinic to be your partner in taking control of your health and well-being. The best way to honor the spirit of ACTCM is to continue to receive acupuncture treatments from your local practitioner.
To assist in that endeavor, the following information will be useful after we close.
Regarding access to your medical and billing records:
Records are kept for 10 years after the last treatment.
After our last on-site clinic shift on August 18, CIIS will be the custodian of your records.
To receive a copy of your records, you must sign a medical release form which will be available to download from our website (www.actcm.edu) and by contacting the Registrar’s Office at CIIS ( #415-575-1267 or registrar@ciis.edu); and pay the copy fee.
Copies are not sent electronically and will be available for pick up at the Registrar’s Office (1453 Mission Street) or mailed to you. Shipping charges, if any, will be determined by CIIS.
If you are hoping to seek out treatments from practitioners you have seen in the past, you can easily search for their names and add “acupuncture” in the search field. Unfortunately, if you do not remember the name of the practitioner you saw, the only way may be to fill out a Google Doc set up to try to stay connected. No contact info will be shared without your permission.
