Big News South LA!

We’re opening a brand-new Habitat Health Center in South Los Angeles in collaboration with Kaiser Permanente Southern California —designed to bring more good days, more support, and more connections to older adults in our communities.

If you’re 55 or older and have both Medicare and Medi-Cal, you may be eligible to receive the following at no cost:

• Medical care (primary & specialty)
• In-home help with daily needs
• Transportation to appointments
• Nutritious meals
• Prescriptions
• A place to connect with others
• One team coordinating it all—with you at the center

Whether you’re helping a loved one or seeking care for yourself, we’re here for
you. Our mission is to support older adults so they can stay independent.

Every detail you see here was made with our participants — their ideas, joy, and imagination are what brighten our center every day.

We’re thankful for the older adults who bring so much life to Habitat Health, the caregivers who support them, and the team who helps create moments like this.

As many of you spend time around your own tables this week — whether you’re crafting, cooking, talking, or simply sharing space — we hope you savor the moments of connection: the conversations, the shared tables, the little pauses that bring people closer. 🧡🍂

During National Family Caregivers' Month, we recognize the essential role family caregivers play in the wellbeing of older adults.

We also recognize that no one should have to walk this path alone.

At Habitat Health, we support caregivers by taking on the daily weight of coordinating care, ensuring safe transportation, keeping seniors active, and being a steady team of providers families can trust.

Because caregivers deserve rest too.

This week the Habitat Health team started distributing supplemental food boxes with 7 days' worth of nutritious staples to help our participants stay nourished and well.

A heartfelt thank-you to our dietitians and staff who quickly came together and, in less than a week, identified participants affected by the disruption of SNAP benefits and packed over 100 food boxes to distribute—demonstrating what community care truly looks like in action.

Last month we reached our 100-participant milestone and today we got to celebrate with the Habitat Health community!

This isn’t just about reaching 100—it’s about celebrating the families who’ve invited us into their lives and trusted us with their health. Since launching in January, we’ve already seen the incredible impact of what happens when compassionate care meets community.

We’re deeply grateful to our care team for their dedication, and to our participants for the spirit of collaboration that makes this milestone possible.
